重组人脑钠肽、沙库巴曲缬沙坦钠和托伐普坦序贯治疗老年AMI后HF患者的临床疗效观察

摘　　要：目的观察冻干重组人脑钠肽、沙库巴曲缬沙坦钠、托伐普坦序贯治疗老年急性心肌梗死后心力衰竭的疗效。方法选择2019年1月~2020年10月收入院的老年急性心肌梗死后心力衰竭患者90例,并随机分为对照组和观察组,每组各45例。对照组在常规治疗的基础上加用冻干重组人脑钠肽、沙库巴曲缬沙坦钠治疗,观察组在对照组的基础上加用托伐普坦治疗,两组分别治疗14 d。监测治疗前后两组心功能、B型脑钠肽(BNP)、血清肾素(PRA)、醛固酮(ALD)、血清钠钾等情况。结果治疗后观察组的LVEDd、LVESd、BNP、PRA、ALD水平较对照组显著减低(P<0.05),LVEF、血钠水平较对照组显著升高(P<0.05),血钾水平无明显改变(P>0.05)。结论应用冻干重组人脑钠肽、沙库巴曲缬沙坦钠、托伐普坦治疗老年急性心肌梗死后心力衰竭,可改善心脏功能,降低神经激素水平,安全性高。Objective To observe the efficacy of lyophilized recombinant human brain natriuretic peptide,salkubatrol valsartan sodium and tovaptan in the sequential treatment of heart failure after acute myocardial infarction in the elderly.Methods 90 elderly patients with heart failure after acute myocardial infarction admitted to the hospital from January 2019 to October 2020 were randomly divided into the control group and the observation group,with 45 cases in each group.The control group was treated with lyophilized recombinant human brain natriuretic peptide and salkubatrol valsartan sodium on the basis of routine treatment,and the observation group was treated with tovaptan on the basis of the control group.The two groups were treated for 14 days respectively.Cardiac function,B-type brain natriuretic peptide(BNP),serum renin(PRA),aldosterone(ALD),serum sodium and potassium were monitored before and after treatment..Results After treatment,the levels of LVEDd,LVESd,BNP,PRA and ALD in the observation group were significantly lower than those in the control group(P<0.05),the levels of LVEF and blood sodium were significantly higher than those in the control group(P<0.05),and the level of blood potassium had no significant change(P>0.05).Conclusion The application of lyophilized recombinant human brain natriuretic peptide,sacubatrovalsartan sodium and tovaptan in the treatment of heart failure after acute myocardial infarction in the elderly can improve cardiac function,reduce the level of neurohormone and have high safety.
