乙型肝炎肝硬化患者外周血IL⁃6水平与肝功能、HBV⁃DNA载量及感染幽门螺杆菌的相关性研究

摘　　要：目的探讨乙型肝炎肝硬化患者外周血白细胞介素-6(interleukin 6,IL-6)水平与肝功能、乙型肝炎病毒的脱氧核糖核酸(HBV-DNA)载量及感染幽门螺杆菌(Helicobacter pylori,HP)的关系。方法选取首都医科大学附属北京世纪坛医院2017年6月至2021年6月收治的126例乙型肝炎肝硬化患者作为研究组,选取同期60例健康人作为对照组。参照Child-Pugh分级标准将研究组患者分为A级(31例)、B级(60例)、C级(35例)。分别检测2组患者外周血IL-6、肝功能、HBV-DNA及HP抗体水平,分析比较不同Child-Pugh分级患者IL-6水平与肝功能、HBV-DNA及HP感染的相关性。结果研究组患者IL-6水平明显高于对照组,且C级>B级>A级(P<0.05);研究组患者谷丙转氨酶(ALT)、谷草转氨酶(AST)明显高于对照组,且C级>B级>A级;研究组患者白蛋白(ALB)明显低于对照组,且C级<B级<A级(P<0.05);研究组患者HBV-DNA明显高于对照组,且C级>B级>A级(P<0.05);研究组患者HP阳性率明显高于对照组(P<0.05),但不同分级患者差异无统计学意义(P>0.05);Pearson分析结果显示,研究组患者IL-6水平与ALT、AST、HBV-DNA、HP阳性等呈显著正相关,与ALB呈显著负相关(P<0.05)。结论乙型肝炎肝硬化患者外周血IL-6水平与其肝功能及HBV-DNA载量、是否感染HP密切相关,可作为评估患者肝功能损伤及HBV感染、HP感染的潜在标志物。Objective To investigate the relationship between IL-6 level and liver function,HBV-DNA load and Helicobacter pylori infection in the patients with hepatitis B liver cirrhosis.Methods A total of 126 hepatitis B cirrhosis patients admitted to Shijitan Hospital from June 2017 to June 2021 were recruited as the study group,while 60 healthy volunteers were selected as the control group.In accordance with the Child-Pugh classification standards,the patients of the study group were further divided into grade A(n=31),grade B(n=60)and grade C(n=35).The levels of IL-6 and liver function,HBV-DNA and HP antibody in the patients of the 2 groups were detected,and the correlation between IL-6 and liver function,HBV-DNA and HP infection was analyze and compared in the patients with different Child-Pugh grades.Results Comparison indicated that the IL-6 levels in the patients of the study group were significantly higher than those of the control group,and grade C>B>A(P<0.05).Further comparison revealed that the levels of ALT and AST in the patients of the study group were significantly higher than those of the control group,and grade C>B>A(P<0.05).Comparison results of ALB showed that the ALB level in the patients of the study group was significantly lower than that of the control group,and grade C>B>A(P<0.05).Comparison results also indicated that the HBV-DNA level in the patients of the study group was significantly higher than that of the control group,and grade C>B>A(P<0.05).Comparison of HP positive rates showed that the positive rate in the patients of the study group was significantly higher than that of the control group(P<0.05),and there were no statistical differences in the patients with different grades(P>0.05);Pearson analysis showed that the IL-6 level was positively correlated with the positive rates of ALT,AST,HBV-DNA and HP,but was negatively correlated with ALB(P<0.05).Conclusion The level of IL-6 in the peripheral blood of the patients with hepatitis B cirrhosis is closely associated with the liver function and
