摘  要:目的探讨心脏磁共振(CMR)T_(1) mapping成像评估左心室延迟强化(LGE)阴性扩张型心肌病(DCM)患者左心室心肌间质纤维化的可行性.方法选取接受CMR检查LGE阴性的DCM患者33例,其中左室射血分数(LVEF)>35%12例.同时纳入40例健康志愿者作为对照组.分析DCM及各亚组患者和对照组间各基线指标、左心室固有T_(1)值和细胞外容积(ECV)之间的差异.结果与对照组相比,DCM患者组左室舒张末期容积、左室收缩末期容积及左房容积指数升高,LVEF减低(P<0.05);DCM患者组固有T_(1)值(1319.91±93.69 vs 1220.65±104.20,P<0.05)和ECV(31.42±3.74 vs 26.25±3.81,P<0.05)均明显升高;在亚组分析中,LVEF<35%DCM患者的ECV明显升高(32.43±3.97 vs 29.67±2.46,P<0.05).结论基于CMR的T_(1) mapping成像能够先于LGE发现DCM患者心肌弥漫性间质纤维化.Objective To explore the feasibility of T_(1) mapping by cardiac magnetic resonance(CMR)in evaluating left ventricular myocardial interstitial fibrosis in dilated cardiomyopathy(DCM)patients without late gadolinium enhancement(LGE).Methods Thirty-three DCM patients who underwent CMR examination and were negative for LGE were enrolled in this study,including 12 cases with left ventricular ejection fraction(LVEF)>35%.Meanwhile,40 healthy volunteers were recruited as the control group.The baseline parameters,left ventricular native T_(1) values,and extracellular volume fraction(ECV)were compared between DCM patients,each subgroup of patients,and control group.Results Compared with control group,DCM patients had higher left ventricular end-diastolic volume,left ventricular end-systolic volume,left atrial volume index,and lower LVEF(P<0.05).The native T_(1)values of DCM patients(1319.91±93.69 vs 1220.65±104.20,P<0.05)and ECV(31.42±3.74 vs 26.25±3.81,P<0.05)were significantly increased.In the subgroup analysis,DCM with LVEF<35% had higher ECV(32.43±3.97 vs 29.67±2.46,P<0.05).Conclusion CMR T_(1) mapping imaging can detect diffuse interstitial fibrosis of the myocardium prior to the presence of myocardial LGE in DCM patients.
