摘 要:目的探讨低剂量联合迭代算法(iDose4)的CT曲面重建(CPR)在输尿管阴性结石中的诊断价值.方法选取高度怀疑输尿管结石而行泌尿系超声及X线检查均为阴性的患者46例,均行256层螺旋CT检查,应用120 kV、200 mAs低剂量扫描(常规剂量为120 kV、400 mAs),行迭代算法(iDose4)薄层轴位重建,后行CPR成像.结果在薄层轴位图像上,46例患者共发现输尿管结石37枚,其中腹段15枚,盆段16枚,膀胱段6枚,9例阴性,阳性诊断率为80.43%,继发不同程度输尿管梗阻34例;在CPR图像上46例患者共发现输尿管结石44枚,其中腹段17枚,盆段20枚,膀胱段7枚,2例阴性,阳性诊断率为95.65%,继发不同程度输尿管梗阻40例.二者阳性诊断率差异有统计学意义(P<0.05).结论低剂量联合迭代算法(iDose4)的CT CPR可以在降低患者辐射剂量1/2左右的前提下清晰显示输尿管结石的位置、大小、数目、形态及继发梗阻表现,优于轴位图像.Objective To investigate the diagnostic value of low dose CT curve plannar reformation(CPR)combined with iterative algorithm(iDose4)in patients with negative ureteral stones.Methods Forty-six patients with high suspicion of ureteral stones and negative urological ultrasound and plain radiographs were recruited and underwent 256-slice spiral CT examination with a low-dose scanof 120 kV,200 mAs(the conventional dose was 120 kV,400 mAs),followed by iterative algorithm(iDose4)thin-layer axial reconstruction,and then CPR imaging.Results On the thin layer axial images,37 ureteral stones were observed in 46 patients,including 15 in the abdominal segment,16 in the pelvic segment,and 6 in the bladder segment,with 9 negative cases and the positive diagnostic rate of 80.43%,and 34 patients had various degrees of secondary ureteral obstruction;on the CPR images,44 ureteral stones were observed in 46 patients,including 17 in the abdominal segment,20 in the pelvic segment,and 7 in the bladder segment,with 2 negative cases and he positive diagnostic rate of 95.65%,and 40 cases had various degrees of secondary ureteral obstruction,and the difference between the two positive diagnostic rates was statistically significant(P<0.05).Conclusion Low-dose CT CPR combined with iterative algorithm(iDose4)can display the location,size,number,morphology,and secondary obstruction of ureteral stones with about half of the radiation dose on patients,it is superior to axial images.
