结石性肾周炎与输尿管结石的大小及尿路梗阻程度的关系

摘　　要：目的探讨结石性肾周炎与输尿管结石的大小及尿路梗阻程度的关系.方法选取输尿管结石453例,根据结石的横截面积分为<10 mm^(2)、10~29 mm^(2)、30~49 mm^(2)、50~69 mm^(2)、≥70 mm^(2)5组,分析各组与尿路梗阻的程度及与肾周炎的关系.结果453例输尿管结石共累及466侧,215侧有肾周炎改变(46.1%),肾积水中轻度335侧、中度63侧及重度68侧.215侧肾周炎中轻度、中度及重度肾积水分别为142侧、41侧及32侧.335侧轻度肾积水的结石横断面积分别为<10 mm^(2)162个、10~29 mm^(2)117个、30~49 mm^(2)37个、50~69 mm^(2)10个、≥70 mm^(2)9个,63侧中度肾积水的结石横断面积分别为<10 mm^(2)6个、10~29 mm^(2)18个、30~49 mm^(2)15个、50~69 mm^(2)7个、≥70 mm^(2)17个,68侧重度肾积水的结石横断面积分别为<10 mm^(2)3个、10~29 mm^(2)10个、30~49 mm^(2)10个、50~69 mm^(2)9个、≥70 mm^(2)36个.结论输尿管结石的大小与肾积水的程度呈正相关性,肾周炎是否存在与输尿管结石的大小及肾积水的程度无明显相关性.Objective To investigate the correlationship of perinephritis with size of ureteral calculi and degree of urinary tract obstruction.Methods 453 cases with ureteral calculi were divided into 5 groups according to the cross sectional area of calculi:<10 mm^(2),10-29 mm^(2),30-49 mm^(2),50-69 mm^(2),≥70 mm^(2),and the relationship between calculi size,degree of urinary tract obstruction and the involvement of perirenal space were analyzed.Results Of the 453 patients,466 sides were involved,and 215 perinephric change were found(46.1%),including mild hydronephrosis(n=335),moderate hydronephrosis(n=63)and severe hydronephrosis(n=68).There were mild hydronephrosis(n=142),moderate hydronephrosis(n=41)and severe hydronephrosis(n=32)in 215 sides perinephric change.There were<10 mm^(2)calculi(n=162),10-29 mm^(2)(n=117),30-49 mm^(2)(n=37),50-69 mm^(2)(n=10)and≥70 mm^(2)(n=9)in 335 sides mild hydronephrosis;<10 mm^(2)calculi(n=6),10-29 mm^(2)(n=18),30-49 mm^(2)(n=15),50-69 mm^(2)(n=7)and≥70 mm^(2)(n=17)in 63 sides moderate hydronephrosis;<10 mm^(2)calculi(n=3),10-29 mm^(2)(n=10),30-49 mm^(2)(n=10),50-69 mm^(2)(n=9)and≥70 mm^(2)(n=36)in 68 sides severe hydronephrosis.Conclusion The size of ureteral calculi is related to the degree of hydronephrosis.There is no significant correlation between perinephritis and the size of ureteral calculi and the degree of hydronephrosis.
