膝关节骨性关节炎疼痛程度与膝关节MRI评分间的相关性研究

摘　　要：目的探讨膝关节骨性关节炎(OA)患者西安大略和麦克马斯特大学骨关节炎指数(WOMAC)疼痛评分与全膝关节磁共振成像评分(WORMS)之间的相关性.方法选取膝关节OA患者90例,均进行X线检查Kellgren-Lawrance(K-L)分级,所有患者均采用3.0T MR对膝关节进行矢状位扫描,对MRI图像采用WORMS法对全膝关节进行全面的评分,并采用WOMAC疼痛评分评估患者膝关节疼痛程度,分析膝关节病理改变与疼痛程度的相关性.结果患者WOMAC疼痛评分平均分为(8.84±4.31)分,WORMS总分平均分为(64.22±11.27)分;年龄、体质量指数(BMI)、软骨、骨髓异常、骨囊肿、骨磨损、骨赘、半月板、滑膜炎与WOMAC疼痛评分均呈正相关(r=0.095、0.103、0.152、0.268、0.192、0.452、0.653、0.337、0.345,P<0.05).结论WORMS与WOMAC疼痛评分具有显著相关性,且WORMS能够对膝关节的病变进行全面的评价,临床可用WORMS判断患者的疼痛来源.Objective To explore the correlation between the pain score of the Western Ontario and McMaster Universities osteoarthritis index(WOMAC)and the whole organ magnetic resonance imaging score(WORMS)in patients with knee osteoarthritis(OA).Methods Ninety patients with knee OA were selected,all patients underwent X-ray examination Kellgren-Lawrance(K L)grading.All patients were used 3.0T MR to perform sagittal scan of the knee joint.The MRI image was analyzed by WORMS method.The whole knee joint was scored comprehensively,and the WOMAC pain score was used to evaluate the knee joint pain of patients,and the correlation between the pathological changes of the knee joint and the pain degree was analyzed.Results The average WOMAC.pain score and WORMS score of all patients were(8.84±4.31)and(64.22±11.27).The WOMAC pain score was positively correlated with age,body mass index(BMI),cartilage,bone marrow abnormalities,bone cysts,bone wear,osteophytes,meniscus,and synovitis(r=0.095,0.103,0.152,0.268,0.192,0.452,0.653,0.337,0.345,P<0.05).Conclusion WORMS has a significant correlation with WOMAC pain score,and WORMS can comprehensively evaluate knee joint lesions,and WORMS can be used to determine the source of pain in patients clinically.
