更年期女性与成年女性中、重度阻塞性呼吸暂停低通气综合征患者睡眠结构比较分析

摘　　要：目的:探讨更年期女性与成年女性阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的睡眠结构特点。方法:选取2014年9月至2020年12月中山市中医院睡眠专科收治的主诉打鼾症状,并经多导睡眠呼吸监测系统(PSG)确诊符合中重度OSAHS的更年期女性患者97例作为观察组,选取同期成年女性27例作为对照组,采用PSG监测2组患者的脑电图、眼动图、颏肌电图、腿肌电图、口鼻呼吸(热敏、气流)、鼾声、胸腹呼吸、体位、经皮血氧饱和度和心电图等指标变化,并比较2组患者睡眠结构的差异。结果:观察组女性N3期潜伏期高于对照组,N3次数、N3持续时间、R期持续时间、R期次数显著低于对照组,2组比较差异均有统计学意义(均P<0.05);REM期的觉醒总次数显著低于对照组,W持续时间显著高于对照组,2组比较差异均有统计学意义(均P<0.05)。结论:更年期女性OSAHS组睡眠结构较成年女性组显示出更多后移性,在睡眠结构中,更年期妇女组NREM睡眠3期、REM睡眠明显减少;更年期女性更容易发生睡眠觉醒。Objective:To investigate the sleep structure of menopausal women and adult women with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:A total of 97 menopausal women with moderate or severe OSAHS who complained of snoring and were confirmed by polysomnography(PSG)from September 2014 to December 2020 in sleep department of Zhongshan Hospital of Traditional Chinese Medicine were selected as the observation grouop,27 adult female were selected as the control group,and PSG was used to monitor the changes of eye movement,mental electromyography,leg electromyography,oral-nasal respiration(heat sensitivity,airflow),snoring,chest and abdominal respiration,body position,transcutaneous oxygen saturation and electrocardiogram,and compare the differences of sleep structure between the 2 groups.Results:The latency of N3 stage in the observation group was higher than that in the control group,and the number of N3,N3 duration,duration of R stage and number of R stage in the observation group were significantly lower than those in the control group,with statistical significance(P s<0.05).The total number of awakening in REM was significantly lower than that in control group,and the W duration was significantly higher than that in control group,with statistical significance(P s<0.05).Conclusion:The sleep structure of menopausal women in OSAHS group is more backward than that of adult women.In the sleep structure,the NREM stage 3 and REM sleep of menopausal women are significantly reduced,and menopausal women are more likely to experience sleep awakenings.
