摘　　要：目的探讨消化道肿瘤患者的癌性厌食水平及其影响因素。方法收集243例消化道肿瘤患者的临床资料。使用肿瘤患者食欲症状问卷(CASQ)、疼痛视觉模拟量表、癌症复发恐惧问卷(FCRI)、医院焦虑抑郁量表(HADS)、中文版癌症疲乏量表(CFS)对患者进行调查。分析消化道肿瘤患者癌性厌食水平与相关心理因素的相关性,采用多元线性回归模型分析消化道肿瘤患者癌性厌食水平的影响因素。结果243例消化道肿瘤患者的CASQ得分为(21.15±3.60)分,得分较低的前3个条目分别为加餐情况、精力状况、味觉变化。不同年龄、性别、临床分期、手术史、疼痛程度、经济负担的消化道肿瘤患者CASQ得分差异均有统计学意义(均P<0.05),年龄为18~35岁、女性、临床分期为Ⅳ期、有手术史、重度疼痛、经济负担很重的患者CASQ得分相对更低。消化道肿瘤患者癌性厌食水平与FCRI得分、HADS A和D亚量表得分均呈负相关(均P<0.05),与CFS得分无相关性(P>0.05)。性别、临床分期、手术史、HADS A和D亚量表得分是消化道肿瘤患者癌性厌食水平的影响因素(均P<0.05)。结论消化道肿瘤患者癌性厌食水平普遍较高,女性、临床分期高、有手术史、焦虑、抑郁的消化道肿瘤患者癌性厌食水平更高。Objective To explore the cancer anorexia level of patients with gastrointestinal tumors and its influencing factors.Methods The clinical data of 243 patients with gastrointestinal tumors were collected.Investigation was conducted on patients using the Cancer Appetite and Symptom Questionnaire(CASQ),Visual Analogue Scale for pain,Fear of Cancer Recurrence Inventory(FCRI),Hospital Anxiety and Depression Scale(HADS),and Cancer Fatigue Scale(CFS)in Chinese version.The correlation of cancer anorexia level of patients with gastrointestinal tumors with related psychological factors was analyzed,and the multiple linear regression model was employed to analyze the influencing factors for cancer anorexia level of patients with gastrointestinal tumors.Results Score of CASQ in 243 patients with gastrointestinal tumors was 21.15±3.60,and 3 items revealed relatively low scores as follows:extra meal condition,energy status,and taste change,respectively.There was statistically significant difference in CASQ score between gastrointestinal tumors patients with different ages,genders,clinical stages,operation histories,pain degree,and economical burden(all P<0.05).The patients aged between 18 to 35 years old,of female gender,with clinical stageⅣ,with prior operation history,with severe pain and heavy economical burden exhibited a relatively lower CASQ score.The cancer anorexia level of patients with gastrointestinal tumors negatively correlated with FCRI score,and HADS A and D subscale scores(all P<0.05),whereas no correlation was found between the level of cancer anorexia and CFS score(P>0.05).Gender,clinical stage,operation history,HADS A and D subscale scores were the influencing factors for the cancer anorexia level of patients with gastrointestinal tumors(all P<0.05).Conclusion The level of cancer anorexia of patients with gastrointestinal tumors is high in general,and gastrointestinal tumors patients of female gender,with high clinical stage,with prior operation history,with anxiety or depression interpret a higher level o
