代谢综合征诱发精子质量异常的研究

摘　　要：目的 探讨代谢综合征(MS)患者糖脂代谢紊乱及性激素紊乱与精子质量异常的关系。方法 根据国际糖尿病联盟(IDF)在2005年新制定的标准,选择2017年9月13日至2020年10月26日就诊于宁夏医科大学总医院泌尿外科男性MS患者79例,参照《WHO人类精液检查与处理实验室手册》(第五版)制定的精液质量正常标准分为MS精子异常组59例,MS精子正常组20例。同期选择门诊婚检的健康男性41例。分析不同组间的空腹血糖(FPG)、胆固醇(CHOL)、甘油三酯(TG)等生化指标,精子存活率、快速向前运动+非快速向前运动精子比率及正常形态精子比率等精液质量参数,雄激素以及雌二醇(E2)、黄体生成素(LH)、卵泡刺激素(FSH)水平等的差异。结果 MS精子异常组和MS精子正常组患者腰围、体质量、收缩压以及舒张压均大于正常对照组(P均<0.05)。MS精子异常组FPG高于MS精子正常组和正常对照组(P<0.05)。精液参数比较中,MS精子异常组精子的活率、快速向前运动+非快速向前运动精子比率及正常形态精子比率等精子参数均低于MS精子正常组以及正常对照组(P均<0.05)。MS精子异常组、MS精子正常组TG、CHOL、LDL水平均高于正常对照组,HDL水平均低于正常对照组(P均<0.05)。MS精子异常组E2、LH、FSH水平均高于正常对照组和MS精子正常组(P均<0.05)。结论 血糖及E2、LH、FSH等激素水平的升高是MS患者精子发生异常的重要原因。Objective To explore the relationship between glucose and lipid metabolism disorder,sex hormone disorder and abnormal sperm quality in patients with metabolic syndrome.Methods According to the new standards established by the International Diabetes Federation(IDF)in 2005,79 cases of male metabolic syndrome patients and outpatient premarital examinations were selected from September 13,2017 to October26,2020 in the Department of Urology,the General Hospital of Ningxia Medical University.Among which men with metabolic syndrome were divided into 59 cases of MS sperm abnormality group and 20 cases of MS normal sperm group according to the WHO Laboratory Manual for Human Semen Examination and Processing 5th edition standard for semen quality parameters.During the same period,41 healthy males who chosed outpatient premarital examination,the biochemical indicators were analyzed such as fasting blood glucose,cholesterol,triglyceride,etc.Between different groups,semen quality parameters such as sperm survival rate,rapid forward motility + non-rapid forward motility sperm ratio and normal morphology sperm ratio,androgen,and E2,LH,Differences in FSH levels were measured.Results The waist circumference,weight,systolic blood pressure and diastolic blood pressure of the MS sperm abnormal group and the MS normal sperm group were higher than those of the normal control group(P all<0.05).The fasting blood glucose in the MS sperm abnormal group was higher than that in the MS normal sperm group and the normal control group(P<0.05).In the comparison of semen parameters,the sperm motility rate,the ratio of fast forward motion + non-fast forward motility sperm,and the ratio of normal sperm in the MS sperm abnormal group were lower than those of the MS normal sperm group and the normal control group(P all<0.05).The levels of TG,CHOL and LDL in the MS sperm abnormal group and MS normal sperm group were higher than those in the normal control group,the levels of HDL were lower than normal control group(P all<0.05).The levels of E2,LH and
