抗ds-DNA抗体及其分泌型B淋巴细胞对系统性红斑狼疮的病情监测

摘　　要：目的 探讨抗ds-DNA抗体及抗ds-DNA抗体分泌型B淋巴细胞在系统性红斑狼疮(systemic lupus erythematosus,SLE)病情监测中的应用价值。方法 选择宁夏回族自治区人民医院2019年1月至2020年10月收治的40例SLE患者为SLE组,同期40例非SLE患者为非SLE组,40例体检结果健康者为健康对照组,对所有研究对象免疫学指标的检测结果进行分析。结果 SLE组患者的抗ds-DNA抗体、抗ds-DNA抗体分泌型B淋巴细胞等检测阳性率均高于非SLE组、健康对照组(P均<0.05);抗ds-DNA抗体、抗ds-DNA抗体分泌型B淋巴细胞的敏感性、阳性预报价值、阴性预报价值高于其他几项抗体诊断价值(P均<0.05);抗dsDNA抗体联合抗ds-DNA抗体分泌型B淋巴细胞的检测特异性及敏感性均优于其他指标(P均<0.05)。结论 抗ds-DNA抗体及抗ds-DNA抗体分泌型B淋巴细胞具有一定的诊断特异性、敏感度,联合检测所获得检测价值更优于其他指标,临床指导价值明显。Objective To investigate the application value of anti-ds-DNA antibody and anti-ds-DNA antibody secreting B lymphocytes in the surveillance of systemic lupus erythematosus.Methods Forty SLE patients admitted to the Ningxia People’s Hospital from January 2019 to October 2020 were selected as the SLE patients group,40 non-SLE patients were selected as the non-SLE patients group,and 40 healthy patients were selected as the healthy control group.The detection results of immunological indexes of the three groups were analyzed.Results The positive rates of anti-ds-DNA antibody and anti-ds-DNA antibody secreting B lymphocytes in the SLE patients group were higher than those in the non-SLE patients group and the healthy control group(P all<0.05).The diagnostic values of anti-ds-DNA antibody and anti-ds-DNA antibody secreted B positive and negative predictive value of secretory B lymphocytes of anti-ds-DNA antibody were higher than other antibodies(P all<0.05).The specificity and sensitivity of anti-ds-DNA antibody combined anti-ds-DNA antibody secretory B lymphocytes were significantly better than other indexes(P all<0.05).Conclusion Anti-ds-DNA antibody and anti-ds-DNA antibody secreting B lymphocytes have certain diagnostic specificity and sensitivity,and the detection value obtained by combined detection is better than other indicators,with obvious clinical guidance value.
