摘　　要：目的探讨自体浓缩生长因子结合A型肉毒毒素治疗男性雄激素性脱发的临床疗效。方法选择我院整形美容门诊和皮肤科脱发门诊男性雄激素性脱发患者32例,于脱发区域设立自身对照,脱发面积等分为两组,A组局部注射A型肉毒毒素,B组联合注射自体浓缩生长因子和A型肉毒毒素;分别于治疗前、治疗后3月和治疗后6月观察毛发密度、终毛/毳毛比例,统计照片客观评价评分、患者满意度、不良事件发生率。结果A组治疗3月后,头发密度、终毛/毳毛比例与治疗前相比较无显著性差异(P>0.05);治疗6月后与治疗前比较,差异具有统计学意义(P<0.05);B组治疗3月和6月后,头发密度、终毛/毳毛比例与治疗前相比较差异均具有统计学意义(P<0.05);A、B两组间在治疗3月和6月后相比较无显著性差异(P>0.05)。治疗3个月后,A、B两组治疗有效率分别为28.13%和40.63%,患者满意度为25.00%和34.38%;治疗6个月后,A、B两组治疗有效率分别为53.13%和78.13%,患者满意度为46.88%和71.88%,且治疗期间两组均无并发症发生。结论CGF联合A型肉毒毒素注射治疗男性AGA疗效显著,安全性高,可以实现微创注射治疗最大化,值得临床推广。Objective To explore the clinical effect of autologous concentrated growth factor combined with botulinum toxin A in the treatment of male androgenetic alopecia.Methods 32 male androgenetic alopecia patients in plastic surgery out-patient departments and dermatology alopecia clinic of our hospital were selected.Self-control was set up in the alopecia area which was divided into two groups,group A was injected with botulinum toxin A locally,group B was injected with autologous concentrated growth factor and Botulinum toxin A.Hair density,terminal hair/vellus hair ratio were observed before treatment,3 months and 6 months after treatment.Objective photographic evaluation score,patient satisfaction and incidence of adverse events were analyzed.Results After 3 months of treatment,there were no significant differences in hair density and V/T hair ratio in group A compared with those before treatment(P>0.05).The difference was statistically significant after 6 months of treatment compared with before treatment(P<0.05).The hair density and vellus/terminal hair ratio were significantly different from those before treatment in group B after 3 months and 6 months of treatment(P<0.05).There was no significant difference between group A and group B after 3 months and 6 months of treatment(P>0.05).After 3 months of treatment,the therapeutic effective rate of group A and B was 28.13%and 40.63%respectively,and the patient satisfaction rate was 25.00%and 34.38%.The therapeutic effective rate of treatment in group A and B was 53.13%and 78.13%respectively,and the patient satisfaction rate was 46.88%and 71.88%after 6 months of treatment.further,there was no complications occurred in the two groups during treatment.Conclusion The treatment of male AGA with CGF combined with botulinum toxin A injection has significant efficacy which is safe without side effects.It can maximize the therapeutic effect of injection therapy and is worthy of clinical promotion.
