基于LC-MS/MS技术探讨GDM合并妊娠期高血压孕妇血清LPC18:0与肾损伤的相关性

摘　　要：目的采用液相色谱-串联质谱(LC-MS/MS)技术检测孕妇血清中1-硬脂酰-sn-甘油-3-磷酰胆碱(LPC18:0)的水平,并探讨其与妊娠期糖尿病(GDM)合并妊娠期高血压孕妇肾功能损伤指标的相关性。方法选择2021年1-12月枣庄市妇幼保健院住院的GDM孕妇120例作为观察组,按照是否合并高血压分为GDM合并妊娠期高血压组(60例)和GDM组(60例),将GDM合并妊娠期高血压组分为LPC18:0≥110μg/mL组(24例)和LPC18:0<110μg/mL组(36例),另选取健康孕妇60例作为对照组。比较各组糖代谢及肾损伤指标的差异,检测GDM合燕妊娠期高血压孕妇血清LPC18:0水平并与肾功能指标进行相关性分析。结果GDM合并妊娠期高血压组与GDM组血清LPC18:0、尿微量白蛋白(UmA1b)、尿微量蛋白/肌酐(ACR)水平比较,差异均有统计学意义(P<0.05)。GDM组与正常对照组血清LPC18:0、空腹葡萄糖(FBG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平比较,差异均有统计学意义(P<0.05)。GDM合并妊娠期高血压组中,LPC18:0≥110μg/mL组与LPC18:0<110μg/mL组UmA1b、ACR水平比较,差异均有统计学意义(P<0.05)。血清LPC18:0水平与BUN、Cr、UmA1b、ACR水平均呈正相关,与FHOMA-IR水平呈负相关(P<0.05)。结论GDM合并妊娠期高血压孕妇血清LPC18:0水平与肾损伤指标存在相关性,可能会成为预测孕妇肾损伤的新型血清指标。Objective To detect the level of 1-stearoyl-Sn-glycerol-3-phosphorylcholine(LPC18:0)in serum of pregnant women by liquid chromatography-tandem mass spectrometry(LC-MS/MS),and investigate its correlation with renal injury indexes in pregnant women with gestational diabetes mellitus(GDM)complicated with gestational hypertension.Methods A total of 120 pregnant women with GDM in hospitalized in Maternity and Child Health Care Hospital of Zaozhuang from January to December 2021 were selected as the observation group.They were divided into the GDM combined with gestational hypertension group(60 cases)and the GDM group(60 cases)according to whether they had hypertension.And the GDM combined with gestational hypertension group was divided into the LPC18:0≥110μg/mL group(n=24)and the LPC18:0<110μg/mL group(n=36),and 60 healthy pregnant women were selected as the control group.The differences of glucose metabolism and renal injury indexes among the two groups were compared.Serum LPC18:0 level in pregnant women with GDM complicated with gestational hypertension was detected and its correlation with renal function indexes were analyzed.Results There were statistically significant differences in serum LPC18:0,urinary microalbumin(UmA1b)and urinary microprotein/creatinine(ACR)levels between the GDM complicated with gestational hypertension group and the GDM group(P<0.05).There were significant differences in serum LPC18:0,fasting glucose(FBG),glycated hemoglobin(HbA1c),fasting insulin(FINS)and insulin resistance index(HOMA-IR)between the GDM group and the control group(P<0.05).In the GDM complicated with gestational hypertension group,the levels of UmA1b and ACR in the LPC18:0≥110μg/mL group and the LPC18:0<110μg/mL group were significantly different(P<0.05).The level of serum LPC18:0 was positively correlated with BUN,Cr,UmA1b and ACR levels,and negatively correlated with FHOMA-IR levels(P<0.05).Conclusion There is a correlation between the level of serum LPC18:0 and renal injury indexes in pregnant women with GDM com
