人工全膝关节翻修术治疗假体无菌性松动的效果研究

摘　　要：目的:分析人工膝关节翻修术治疗假体无菌性松动的效果,对比部分翻修与全部翻修的疗效差异。方法:选取2020年1月-2022年1月15例织金县人民医院人工全膝关节置换术后出现假体无菌性松动患者15例。所有患者均实施翻修术,并根据翻修程度将患者分为部分翻修组(6例)与全部翻修组(9例)。比较不同翻修类型手术相关指标及临床指标。结果:手术后,所有患者美国膝关节协会评分(KSS)均提高,而视觉模拟评分法(VAS)评分均降低,且双下肢差值更小,差异具有统计学意义(P<0.05)。两组术中出血量、引流量及输血量、术后KSS评分、VAS评分比较,差异无统计学意义(P>0.05);部分翻修组手术时间更短,血红蛋白(Hb)减少量更小,差异具有统计学意义(P<0.05)。结论:膝关节翻修术治疗假体无菌性松动的近期疗效显著,且保留部分稳定膝关节假体的部分翻修术临床应用价值更高,能有效缩短手术时间,降低患者Hb丢失量,提高手术安全性,值得推广。Objective:To analyze the effect of artificial knee joint revision in the treatment of aseptic loosening of prosthesis,and compare with the efficacy of partial revision and total revision.Methods:From January 2020 to January 2022,15 patients with aseptic prosthesis loosening after artificial total knee arthroplasty in Zhijin County People's Hospital were selected.All patients underwent a revision surgery and were divided into partial revision group(6 cases)and total revision group(9 cases)according to the degree of revision.The related indexes and clinical indexes of different revision types of surgery were compared.Results:After surgery,the American Knee Society Score(KSS)was improved in all patients,while the Visual Analogue Scale(VAS)score was decreased,and the difference between the smaller extremities was shorter,with statistically significant differences(P<0.05).There was no significant difference in intraoperative blood loss,drainage volume and blood transfusion volume,postoperative KSS score,and VAS score between the two groups(P>0.05);the partial revision group had a shorter operation time and a smaller reduction in hemoglobin(Hb),and the difference was statistically significant(P<0.05).Conclusion:The short-term efficacy of knee joint revision surgery for aseptic loosening of prosthesis is remarkable,and the partial revision surgery with partial stable knee joint prosthesis reserved has a higher clinical application value,which can effectively shorten the operation time,reduce the loss of Hb in patients,and improve the operation safety.It is worth promoting.
