右美托咪定辅助椎管内麻醉对下肢手术患者的镇静效果分析

摘　　要：目的:探讨右美托咪定辅助椎管内麻醉对下肢手术患者的镇静效果。方法:选取2020年1月-2021年12月溧阳市人民医院收治的82例择期行下肢手术的患者作为研究对象,随机分为对照组和观察组,各41例。观察组应用右美托咪定辅助麻醉,对照组应用同量的生理盐水,比较分析两组患者用药前(T_(0))、用药10 min(T_(1))、用药30 min(T_(2))、用药60 min(T_(3))和手术结束(T_(4))的听觉诱发电位指数(AAI)、镇静效果(Ramsay评分)以及不良反应发生率。结果:两组T_(0)、T_(2)、T_(3)时AAI值比较,差异无统计学意义(P>0.05);观察组T_(1)和T_(4)时的AAI值低于对照组,差异有统计学意义(P<0.05)。两组T_(0)时Ramsay评分比较,差异无统计学意义(P>0.05);观察组T_(1)~T_(4)期间的Ramsay评分均高于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:在椎管内麻醉期间加用右美托咪定能够提高麻醉效果和镇静效果,且安全性较高,值得临床推广实践。Objective:To investigate the sedation effect of dexmedetomidine-assisted intraspinal anesthesia in patients undergoing lower limb surgery.Methods:A total of 82 patients undergoing elective lower limb surgery who were admitted to Liyang People's Hospital from January 2020 to December 2021 were selected as the research subjects,and randomly divided into the control group and the observation group,with 41 cases in each group.The observation group was given dexmedetomidine assisted anesthesia,and the control group was given the same amount of normal saline.The two groups were compared and analyzed for auditory evoked potential index(AAI)before medication(T_(0)),10 minutes after medication(T_(1)),30 minutes after medication(T_(2)),and 60 minutes after medication(T_(3))and at the end of surgery(T_(4)),sedation effect(Ramsay score)and incidence of adverse reactions.Results:There was no significant difference in AAI values at T_(0),T_(2),and T_(3) between the two groups(P>0.05).The AAI values at T_(1) and T_(4) in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in Ramsay score between the two groups at T_(0)(P>0.05);the Ramsay score in the observation group during T_(1)-T_(4) was higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The addition of dexmedetomidine during spinal anesthesia can improve the effect of anesthesia and sedation,and has high safety,which is worthy of clinical practice.
