老年晚期肿瘤患者预先指示态度的调查研究

摘　　要：目的应用中文版预先指示态度量表调查老年晚期肿瘤患者对预先指示的态度现状并分析其影响因素。方法方便抽样法选取天津市某三级甲等医院的206例老年晚期肿瘤住院患者为研究对象,采用预先指示态度量表对其进行调查。结果老年晚期肿瘤患者的预先指示态度得分为[39.00(38.00,45.00)]分,治疗选择的机会维度平均得分最高[16.00(15.00,19.00)分]。预先指示的知晓率为14.08%。了解预先指示概念、流程及意义后,67.96%的老年晚期肿瘤患者表示愿意制定预先指示。多元线性回归分析显示,老年晚期肿瘤患者的宗教信仰、婚姻状况及生命意义感是影响其预先指示态度得分的主要因素(均P<0.05)。结论老年晚期肿瘤患者对预先指示的态度处于中等偏上水平,对医疗照护希望有自主选择权。医务人员应充分了解老年晚期肿瘤患者的宗教信仰、婚姻等基本信息并尊重其个人意愿,提供个性化的医疗照护服务。Objective To investigate the status quo of advanced cancer patients’attitudes to advance directives(ADs)and analyze its influencing factors by using Chinese version of Advance Directives Attitude Scale.Methods A questionnaire survey of 206 elderly in-patients with advanced cancer in a tertiary A hospital in Tianjin was conducted by using the convenience sampling method.Results The ADs attitude score of elderly patients with advanced cancer was[39.00(38.00,45.00)],and the average score of the opportunity of treatment choice dimension was[16.00(15.00,19.00)]which was the highest score.The awareness rate of ADs was 14.08%,and the proportion of elderly patients with advanced cancer who expressed their willingness to make ADs after preliminary understanding the concept,process and meaning of the ADs was 67.96%.Multiple linear regression analysis showed that the religious beliefs,marital status,and sense of life meaning of elderly patients with advanced cancer were the main influencing factors of ADs attitudes(all P<0.05).Conclusions Elderly patients with advanced cancer have a moderate to upper level attitude towards ADs,and they show the willingness to have the right to choose their own medical care.Elderly patients with advanced cancer who have religious beliefs,are divorced or widowed,and focus on quality of life have a more positive attitude towards advance directives.Medical staff should fully understand the religion,marriage and other basic information of elderly patients with advanced cancer,respect their personal wishes,and provide personalized medical care.
