摘 要:目的研究右归饮对激素性股骨头坏死(steroid-induced avascular necrosis of femoral head,SANFH)模型大鼠的影响,探讨其作用机制。方法取50只SD大鼠按随机数字表法分为正常组、模型组及右归饮低(6.6 g生药/kg)、中(13.2 g生药/kg)、高(26.4 g生药/kg)剂量组,每组10只。除正常组外,其余各组大鼠采用脂多糖(LPS)联合激素法制备SANFH模型。右归饮各剂量组在造模同时灌胃相应剂量的右归饮,模型组和正常组灌胃等体积生理盐水,1次/d,连续8周。末次给药后,采用全自动生化分析仪测定各组大鼠血清钙、磷水平;各组大鼠股骨头标本行MRI检测,采用HE染色观察大鼠股骨头病理学改变,采用RT-PCR检测大鼠股骨头caspase-3、β-catenin、Wnt3α基因表达。结果与模型组比较,右归饮中、高剂量组血清钙、磷水平升高(P<0.05或P<0.01),右归饮低剂量组血清磷水平升高(P<0.05);右归饮中、高剂量组股骨空骨陷窝率降低(P<0.05或P<0.01);右归饮低、中、高剂量组caspase-3 mRNA[(2.146±0.191)、(1.688±0.247)、(1.370±0.252)比(2.535±0.236)]水平降低(P<0.05或P<0.01),β-catenin mRNA[(0.433±0.102)、(0.496±0.091)、(0.698±0.089)比(0.259±0.106)]水平升高(P<0.05或P<0.01);右归饮中、高剂量组Wnt3αmRNA[(0.509±0.061)、(0.833±0.053)比(0.384±0.052)]水平升高(P<0.05或P<0.01)。右归饮中、高剂量组MRI检测可见关节周少许T2高信号,关节内可见T2高信号,与股骨头软骨界限较分明,股骨头内部无明显异常信号;右归饮中、高剂量组HE染色可见骨小梁粗大,排列较规则,骨细胞大部分正常,空骨陷窝较少。结论右归饮对SANFH大鼠的保护作用可能与抑制Wnt/β-catenin信号通路的激活有关。Objective To investigate the effect of Yougui Yin on steroid-induced femoral head necrosis in rats and the regulation of Wnt/β-catenin signaling pathway,and to explore its mechanism.Methods Fifty SD rats were randomly divided into normal group,model group,high-dose group(26.4 g/kg),medium-dose group(13.2 g/kg)and low-dose group(6.6 g/kg)of Yougui Yin,with 10 rats in each group.Except the normal group,the other groups were prepared with SANFH model by combining LPS and methylprednisolone injection.The treatment groups were intragastrically administered with Yougui Yin,once a day for 8 weeks.After the final administration,the serum calcium and phosphorus levels of rats in each group were determined by automatic biochemical analyzer.The femoral head specimens of rats in each group were detected by MRI,and the pathological changes of the femoral head were observed by HE staining.The expressions of caspase-3,β-catenin and Wnt3αmRNA in the femoral head of rats were detected by RT-PCR.Results Compared with the model group,the levels of serum calcium and phosphorus in the medium and high dose groups were increased significantly(P<0.05,P<0.01),and the level of serum phosphorus in the low dose group was increased significantly(P<0.05);In the medium and high dose groups,the femoral empty bone lacuna rate was decreased(P<0.05,P<0.01);The expression of caspase-3 mRNA(2.146±0.191,1.688±0.247,1.370±0.252 vs.2.535±0.236)in the low,medium and high dose groups were decreased(P<0.05,P<0.01),and the expression ofβ-catenin mRNA(0.433±0.102,0.496±0.091,0.698±0.089 vs.0.259±0.106)were increased(P<0.05,P<0.01);The expression of Wnt3αmRNA(0.509±0.061,0.833±0.053 vs.0.384±0.052)in the medium and high dose groups were increased(P<0.05,P<0.01);In the medium and high dose groups,MRI showed higher T2 signals around the joint,and high T2 signals within the joint,which were clearly distinguished from the femoral head cartilage,and there was no obvious abnormal signal within the femoral head.HE staining in the Yougui Yin medium a
