齿形结扎线形切剥微创术治疗Ⅲ、Ⅳ度静脉曲张型环状混合痔的临床效果

摘　　要：目的 研究齿形结扎线形切剥微创术治疗Ⅲ、Ⅳ度静脉曲张型环状混合痔的临床效果。方法 选取2019年2月至2021年2月本院收治的50例Ⅲ、Ⅳ度静脉曲张型环状混合痔患者为研究对象,按照随机数字表法将其分为对照组(25例,分段齿形结扎术)和观察组(25例,齿形结扎线形切剥微创术)。比较两组的治疗效果。结果 术后4周,观察组的肛管静息压、肛管最大收缩压低于对照组(P<0.05)。术后24、48、72 h,观察组的视觉模拟评分法(VAS)评分低于对照组(P<0.05)。观察组的肛门失禁、肛门狭窄总发生率低于对照组(P<0.05)。观察组的治疗总有效率高于对照组(P<0.05)。两组的并发症总发生率比较,差异无统计学意义(P>0.05)。术后3个月,观察组的简明健康调查量表(SF-36)各维度评分高于对照组(P<0.05)。结论 齿形结扎线形切剥微创术治疗Ⅲ、Ⅳ度静脉曲张型环状混合痔的效果显著,有利于提高患者的健康状态。Objective To study the clinical effect of minimally invasive surgery of linear incision with dentate ligation in the treatment of circular mixed hemorrhoids of varicose veins in stage Ⅲ and Ⅳ.Methods Fifty patients with circular mixed hemorrhoids of varicose veins in stage Ⅲ and Ⅳ treated in our hospital from February 2019 to February 2021 were selected as the research objects.According to the random number table method,the patients were divided into control group(25 cases,segmental odontoid ligation) and observation group(25 cases,minimally invasive surgery of linear incision with dentate ligation).The therapeutic effects of the two groups were compared.Results At 4 weeks after operation,the anal resting pressure and anal maximal squeeze pressure in the observation group were lower than those in the control group(P0.05).At 3 months after operation,the scores of each dimension of Short Form 36Health Survey Scale(SF-36) in the observation group were higher than those in the control group(P<0.05).Conclusion The minimally invasive surgery of linear incision with dentate ligation in the treatment of circular mixed hemorrhoids of varicose veins in stage Ⅲ and Ⅳ has a significant effect,which is conducive to improving the health status of patients.
