散结镇痛胶囊联合腹腔镜手术治疗子宫内膜异位症的效果及对血管生长因子、局部血流情况的影响

摘　　要：目的 探讨散结镇痛胶囊联合腹腔镜手术治疗子宫内膜异位症的效果及对血管生长因子、局部血流情况的影响。方法 选择2018年2月至2020年8月我院收治的140例子宫内膜异位症患者为研究对象,根据治疗方案将其分为观察组和对照组,每组70例。对照组采取腹腔镜手术,观察组在对照组基础上加施散结镇痛胶囊。比较两组的临床疗效、血管生长因子、子宫动脉血流动力学指标、可溶性细胞间黏附分子(sICAM-1)及可溶性白细胞介素2受体(sIL-2R)水平。结果 观察组的治疗总有效率为95.71%,显著高于对照组的84.29%(P<0.05)。治疗后,两组的血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)和转化生长因子β_(1)(TGF-β_(1))水平均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组的舒张末期血流速度(EDV)、收缩期峰值血流速度(PSV)、阻力指数(RI)、搏动指数(PI)均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,两组的sICAM-1、sIL-2R水平均低于治疗前,且观察组低于对照组(P<0.05)。结论 散结镇痛胶囊联合腹腔镜手术治疗子宫内膜异位症的效果显著,能调节机体血管生成因子水平,改善子宫动脉血流动力学指标及sICAM-1、sIL-2R水平。Objective To explore the effect of Sanjie Zhentong capsule combined with laparoscopic surgery on endometriosis and its influences on vascular growth factors and local blood flow.Methods A total of 140 patients with endometriosis treated in our hospital from February 2018 to August 2020 were selected as the research objects and divided into observation group and control group according to the treatment plan,with 70 cases in each group.The control group was treated with laparoscopic surgery,and the observation group was treated with Sanjie Zhentong capsule on the basis of the control group.The clinical efficacy,vascular growth factors,uterine artery hemodynamic indexes,soluble intercellular adhesion molecule-1(sICAM-1) and serum interleukin 2 receptor(sIL-2R) levels were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.71%,which was significantly higher than84.29% in the control group(P <0.05).After treatment,the levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF) and transforming growth factor-β_(1)(TGF-β_(1)) in the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).After treatment,the end diastolic velocity(EDV),peak systolic velocity(PSV),resistance index(RI) and pulsatility index(PI) of the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).After treatment,the levels of sICAM-1 and s IL-2R in the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).Conclusion Sanjie Zhentong capsule combined with laparoscopic surgery in the treatment of endometriosis has a significant effect,which can adjust the levels of angiogenesis factors,and improve the hemodynamic indexes of uterine artery and the levels of sICAM-1 and sIL-2R.
