摘 要:目的 探讨新型壳聚糖止血材料对大鼠股动脉出血模型的止血效果。方法 选取20只健康SD大鼠随机分为新型壳聚糖止血材料组、商品壳聚糖材料组、医用明胶海绵组和医用纱布组,每组各5只。将SD大鼠麻醉,剥离单侧股动脉,在股动脉根部做一小切口,切口长度为动脉直径的1/3,制作动脉出血模型。自由出血30 s后,将4种止血材料分别覆盖在伤口表面,其上覆盖一100 g砝码压迫止血。记录大鼠基础体质量、基础心率、自由出血量、止血失血量、自由出血后心率、止血完成后心率等。结果 新型壳聚糖止血材料、医用明胶海绵、商品壳聚糖材料以及医用纱布的止血成功率分别为60.0%(3/5)、60.0%(3/5)、80.0%(4/5)以及40.0%(2/5),4组比较,差异无统计学意义(P>0.05)。新型壳聚糖止血材料组、商品壳聚糖材料组及医用明胶海绵组的止血失血量均明显少于医用纱布组,止血完成后心率明显高于医用纱布组,差异有统计学意义(P<0.05);新型壳聚糖止血材料组、商品壳聚糖材料组及医用明胶海绵组止血失血量、止血完成后心率比较,差异均无统计学意义(P>0.05)。结论 新型壳聚糖止血材料对大鼠股动脉出血性损伤具有较好的止血效果,且具有稳定心率的作用。Objective To study the hemostatic effect of new chitosan hemostatic material on rat femoral artery bleeding model.Methods Twenty healthy SD rats were randomly divided into new chitosan hemostatic material group,commercial chitosan material group,medical gelatin sponge group and medical gauze group,with 5 rats in each group.SD rats were anesthetized,unilateral femoral artery was dissected,and a small incision was made at the femoral artery root,the length of the incision was 1/3 of the artery diameter,and the model of arterial hemorrhage was made.After 30 s of free bleeding,4 kinds of hemostatic materials were respectively covered on the surface of the wound,and 100 g weight was covered to stop bleeding.Basal body weight,basal heart rate,free blood loss,hemostatic loss,heart rate after free bleeding and heart rate after hemostasis were recorded.Result The success rate of hemostasis was 60.0%(3/5),60.0%(3/5),80.0%(4/5)and 40.0%(2/5)of the new chitosan hemostatic material,medical gelatin sponge,commercial chitosan material and medical gauze,respectively,and there was no statistical significance among the four groups(P>0.05).The hemostatic loss in the new chitosan hemostatic material group,commercial chitosan material and medical gelatin sponge group was significantly lower than that in the medical gauze group,and the heart rate after hemostasis was significantly higher than that in the medical gauze group,with statistical significance(P<0.05).There were no significant differences in hemostatic loss and heart rate after hemostasis between the new chitosan hemostatic material group,commercial chitosan material group and medical gelatin sponge group(P>0.05).Conclusion The new chitosan hemostatic material has a good hemostatic effect on hemorrhagic injury of femoral artery in rats,and has the effect of stabilizing heart rate.
