团体献血制备的冷沉淀凝血因子质量分析

摘　　要：目的 研究团体献血采集的全血在不同时间、不同温度下保存,制备成新鲜冰冻血浆及冷沉淀凝血因子的质量。方法 以2021年4月血站内团体献血采集的血液为研究对象,选取30袋300 ml全血。每袋300 ml全血立即均分为3份,3组,每组30袋。分别保存在(4±2)℃贮血冰箱(对照组)、(18±2)℃室内(实验1组)和(22±2)℃室内(实验2组)。每组取10袋分别于6 h、8 h、10 h运回成分制备科。成分制备科接收血液后在1 h内将全血分离制备成新鲜冰冻血浆。2周后将新鲜冰冻血浆取出,采用虹吸法1~6℃水浴制备成冷沉淀凝血因子。结果 全血在(18±2)℃和(22±2)℃保存6 h、8 h和10 h制备成新鲜冰冻血浆Ⅷ因子含量和冷沉淀凝血因子纤维蛋白原含量均符合国标要求。全血(22±2)℃保存10 h制备成冷沉淀凝血因子Ⅷ因子含量不符合国标要求。结论 为保证冷沉淀凝血因子的质量,团体献血在(18±2)℃保存的全血应10 h内运回,(22±2)℃保存的全血应8 h内运回。Objective To determine the quality of freshly frozen plasma and cryoprecipitate coagulation factors prepared from group-donated blood that was stored for different times and at different temperatures.Methods This study utilized thirty 300 ml bags of whole blood collected from group donations at a blood donation station in April 2021. The bags were divided into three groups of 30 bags each,and each individual bag was divided into three equal portions. They were stored in a blood storage refrigerator at(4±2)℃ (Control Group),indoors at(18±2)℃ (Experimental Group 1)and indoors at(22±2)℃ (Experimental Group 2). Ten bags from each group were transported back to the blood components preparation department at 6 h, 8 h, and 10 h. The blood components preparation department separated the whole blood into freshly frozen plasma within 1 h of receiving the blood. Freshly frozen plasma was removed after 2 weeks of storage cryoprecipitate coagulation factors prepared by siphon method in a 1 ℃-6 ℃ water bath. Results Whole blood was stored at(4±2)℃,(18±2)℃,and(22±2)℃ for 6 h,8 h and 10 h,respectively. The factor Ⅷ content in the fresh frozen plasma and the fibrinogen content in the cryoprecipitate coagulation factors met the national standard. However,the factor Ⅷ content in the cryoprecipitate coagulation factors prepared from whole blood stored at(22±2) ℃ for 10 h did not meet the national standard. Conclusion To ensure the quality of cryoprecipitate coagulation factors,group-donated whole blood stored at(18 ±2) ℃ and(22 ±2) ℃ should be transported within 10 h and 8 h,respectively.
