刘健治疗反应性关节炎用药规律的数据挖掘研究

摘　　要：目的:应用数据挖掘技术和关联规则分析刘健教授中医药治疗反应性关节炎用药规律。方法:基于数据挖掘技术,对2012年6月—2020年3月刘健教授诊治的159例反应性关节炎患者进行回顾性研究,采用SPSS 22.0与SPSS Clementine 11.1进行药物及其性味归经频数分析、聚类分析及关联规则。结果:①药物性味归经前三位:寒性、温性和平性,苦味、甘味和辛味以及归脾、肺、肝经胃经药物使用最频繁。②刘健教授常用药物一般分为4类:清热解毒药、健脾化湿药、活血化瘀药、通络止痛药。③聚类分析得出常用药物可聚为4类,分别为:蒲公英、茯苓、丹参、薏苡仁、陈皮为1类;山药、白术、白花蛇舌草、豨莶草、紫花地丁;透骨草、徐长卿、川牛膝、忍冬藤、连翘、土茯苓1类;鸡血藤、威灵仙、桃仁、红花。④药物之间关联规则分析得出清热解毒、健脾化湿祛风除痹、活血化瘀药是相互关联的。⑤药物与实验室指标之间关联规则分析得出蒲公英关联于IgG、hs-CRP下降关联度较高,茯苓关联于IgA、IgG、hs-CRP、ESR下降关联度较高,薏苡仁关联于IgA、IgG、hs-CRP下降关联度较高,陈皮关联于ESR、IgG、hs-CRP下降关联度较高。⑥随机行走分析得出,中药联合特色制剂对反应性关节炎患者IgG、CRP的改善优于单纯中药组。结论:刘健教授治疗反应性关节炎以清热解毒药为主,配伍健脾化湿药、祛风除痹药、活血化瘀药,在改善hs-CRP、ESR、IgA、IgG等方面具有显著疗效。Objective:to analyze Professor Liu Jian’s medication rule of traditional Chinese medicine in the treatment of reactive arthritis by using data mining technology and association rules.Methods:Based on data mining technology,159 patients with reactive arthritis treated by Professor Liu Jian from June 2012 to March 2020 were retrospectively studied.SPSS 22.0 and SPSS Clementine 11.1 were used for frequency analysis,cluster analysis and association rules of drugs and their properties and flavors.Results:① the top three types of drugs were cold,warm and peaceful,bitter,sweet and spicy,and the drugs for spleen,lung,liver and stomach meridian were used most frequently.② Professor Liu Jian’s commonly used drugs are generally divided into four categories:antipyretic and antidotal drugs,Jianpi Huashi drugs,Huoxue Huayu drugs,Tongluo analgesic drugs.③ Cluster analysis showed that the commonly used drugs could be divided into four categories:dandelion,Poria cocos,Salvia miltiorrhiza,coix lachryma and Pericarpium Citri Reticulatae;Dioscorea opposita,Atractylodes macrocephala,Hedyotis diffusa,Siegesbeckia,Viola;Tougucao,Xu Changqing,chuanniuxi,honeysuckle vine,Forsythia suspensa,pachymenia glabra;Caulis Spatholobus,Clematis Clematis,peach kernel,safflower.④ According to the analysis of association rules between drugs,the drugs of clearing heat and detoxification,invigorating spleen and removing dampness,dispelling wind and removing arthralgia,and promoting blood circulation to remove blood stasis are interrelated.⑤ According to the analysis of association rules between drugs and laboratory indicators,dandelion was associated with the decrease of IgG and hs CRP,Poria cocos was associated with the decrease of IgA,IgG,hs CRP and ESR,coix seed was associated with the decrease of IgA,IgG and hs CRP,and Pericarpium Citri Reticulatae was associated with the decrease of ESR,IgG and hs CRP.⑥ Random walk analysis showed that the improvement of IgG and CRP in patients with reactive arthritis was better than that in the tr
