摘　　要：目的:探讨隐球菌荚膜多糖抗原胶体金免疫层析法(Crypto-coccalantigen lateral flow immunoassay,CrAgLFA)诊断隐球菌性脑膜炎的早期临床价值。方法:收集2019年9月—2021年10月赣南医学院第一附属医院27例确诊为隐球菌性脑膜炎患者的临床资料,选取33例同期经临床确诊为细菌性脑膜炎、结核性脑膜炎或脑部肿瘤等非隐球菌性脑膜炎患者为对照组,应用CrAg-LFA检测隐球菌荚膜多糖抗原,并与真菌培养法和墨汁染色法比较,评价其在隐球菌脑膜炎诊断中的应用价值。结果:CrAg-LFA、真菌培养法、墨汁染色法检测隐球菌的灵敏度分别为96.30%、70.37%、37.04%,特异度分别为96.97%、100.00%、100.00%。结论:CrAg-LFA是一种简便快速的筛查方法,对早期诊断隐球菌性脑膜炎具有重要临床价值。Objective:To explore the early clinical value of cryptococcal capsular polysaccharide antigen with collidal gold immunochromatography in diagnosis of cryptococcal meningitis.Methods:The cryptococcal capsular polysaccharide antigen were tested by cryptococcal antigen-colloidal gold immunochromatography(CrAg-LFA)in cerebrospinal fluid samples of 27 patients with cryptococcal meningitis,and 33 patients(as control group)with bacterial meningitis,tuberculous meningitis or brain tumors of non-cryptococcal meningitis,compared with the fungal culture and Indian ink staining,to evaluate its value in the diagnosis of cryptococcal meningitis.The cases collected were from September 2019 to Octoer2021 in The First Affiliated Hospital of Gannan Medical University.Results:The sensitivity of CrAg-LFA,fungal culture and Indian ink staining was 96.30%,70.37% and 37.04% respectively,the specificity was 96.97%,100.00%and 100.00% respectively.Conclusion:CrAg-LFA is a kind of simple and rapid screening method with important clinical value for the early diagnosis of cryptococcal meningitis.
