尿全项自动分析仪在尿液红细胞、白细胞、管型检测的临床价值

摘　　要：目的探讨尿液红细胞、白细胞、管型应用尿全项自动分析仪检测的临床价值。方法选取2019年8月—2020年7月济南市历城区中医医院检验科接收的160份尿液标本,分别进行尿全项自动分析仪检测及人工显微镜检查,观察对比两种检测方法检测红细胞、白细胞、管型的结果差异性。结果尿全项自动分析仪检测白细胞的灵敏度、特异度分别为96.15%、97.01%,总符合率为96.87%,Kappa值为0.817,说明两种检测方法的一致性较好;尿全项自动分析仪检测红细胞的灵敏度为96.42%,特异度为96.21%,总符合率为96.25%,Kappa值为0.826,说明两种检测方法的一致性较好;尿全项自动分析仪检测管型的灵敏度为93.33%,特异度为97.93%,总符合率为97.50%,Kappa值为0.809,说明两种检测方法的一致性较好。结论尿全项自动分析仪检测操作简单,对白细胞、红细胞、管型检测的敏感性与特异性均较好,兼具警告提示与人工辅判功能,实景图像的人工审核有效降低了误差的发生率,有助于为临床提供更为准确、客观的尿液检验报告。Objective To investigate the clinical value of urine erythrocytes,leukocytes and casts using automatic urine analyzer.Methods A total of 160 urine specimens received by the laboratory department of Jinan Licheng Dis⁃trict Traditional Chinese Medicine Hospital from August 2019 to July 2020 were selected for automatic urine ana⁃lyzer testing and manual microscopy.To observe and compare the differences in the results of the two detection meth⁃ods in detecting red blood cells,white blood cells and casts.Results The sensitivity and specificity of urine total item automatic analyzer in detecting leukocytes were 96.15%and 97.01%respectively,the total coincidence rate was 96.87%,and the Kappa value was 0.817,indicating that the two detection methods had good consistency.The sensitivity and specificity of urine total-item automatic analyzer in detecting red blood cells were 96.42%,96.21%,the total coincidence rate was 96.25%,and the Kappa value was 0.826,indicating that the two detection methods had good consistency.The sensitivity and specificity of the urine total-item automatic analyzer for detecting casts were 93.33%,97.93%,the total coincidence rate was 97.50%,and Kappa value was 0.809,indicating that the two detec⁃tion methods had good consistency.Conclusion The automatic urine analyzer is easy to operate,has good sensitivity and specificity for the detection of white blood cells,red blood cells,and casts.It has both warning prompts and manual auxiliary judgment functions.The manual review of real images effectively reduces the occurrence of errors.It is helpful to provide a more accurate and objective urine test report for the clinic.
