不同麻醉方法对老年髋关节手术患者麻醉起效时间、维持时间及麻醉平面的影响

摘　　要：目的探析对老年髋关节手术患者分别采取硬膜外麻醉、腰麻-硬膜外联合麻醉方式的临床效果。方法选取2020年1月—2021年6月山东第一医科大学附属成武医院收治的老年髋关节手术治疗患者100例,以随机数表方式将其分为A组、B组,每组50例,分别在术中采取硬膜外麻醉、腰麻-硬膜外联合麻醉,比较两组患者麻醉指标差异。结果B组麻醉起效时间(3.27±1.02)min短于A组,麻醉维持时间(228.19±28.17)min长于A组,麻醉平面(9.88±0.31)节段高于A组,差异有统计学意义(t=17.397、20.057、15.033,P<0.05);两组患者血压、心率比较,差异无统计学意义(P>0.05);B组在T1、T2、T3时间点的应激指标水平均高于A组,差异有统计学意义(P<0.05)。结论腰麻-硬膜外联合麻醉方式用于老年髋关节手术不仅麻醉效果突出,且不会影响患者围术期体征稳定性,值得临床推广。Objective To explore the clinical effect of epidural anesthesia and combined spinal and epidural anesthe⁃sia in elderly patients with hip joint surgery.Methods A total of 100 elderly patients with hip surgery were selected in Chengwu Hospital Affiliated to Shandong First Medical University from January 2020 to June 2021,and they were divided into group A and group B by random number table,50 cases in each group.During the operation,epidural an⁃esthesia and combined spinal and epidural anesthesia were used,respectively,and the differences in anesthesia in⁃dexes were compared between the two groups.Results The onset time of anesthesia in group B was(3.27±1.02)min shorter than that in group A,the anesthesia maintenance time was(228.19±28.17)min longer than that in group A,and the level of anesthesia was(9.88±0.31)segments higher than that in group A,and the difference was statistically significant(t=17.397,20.057,15.033,P<0.05);there was no statistically significant difference in the blood pressure and heart rate between the two groups(P>0.05);the stress index levels of group B at T1,T2,and T3 time points were higher than those of group A,and the difference was statistically significant(P<0.05).Conclusion Combined spinal anesthesia-epidural anesthesia for elderly hip surgery not only has outstanding anesthesia effect,but also does not af⁃fect the stability of patients'perioperative signs,which is worthy of clinical promotion.
