摘 要:目的 研究低出生体质量儿血流动力学异常动脉导管未闭(hsPDA)治疗与重度脑室内出血的相关性。方法 回顾性分析62例hsPDA伴或不伴脑室内出血(IVH)低出生体质量儿,按照是否口服布洛芬悬浮液治疗hsPDA分为治疗组和未治疗组,1个疗程治疗结束后立即复查床旁超声心动图和颅脑超声,观察导管关闭和颅脑情况。结果 使用布洛芬悬浮液口服治疗的30例hsPDA低出生体质量儿治疗后MCA的Vs和Vd值较治疗前均增加,RI值降低(P<0.05)。未治疗组的32例hsPDA低出生体质量儿重度IVH发生率(37.5%)占比明显高于30例治疗组(13.3%)(P<0.05)。结论 hsPDA治疗可以改善低出生体质量儿脑血流情况,可以一定程度上防止脑室内出血的进展和重度脑室内出血的发生。Objective To investigate the relationship between hemodynamic abnormality of patent ductus arteriosus(hsPDA) and severe intraventricular hemorrhage in low birth weight infants. Methods 62 cases of low birth weight infants with hsPDA with or without intraventricular hemorrhage(IVH) were retrospectively analyzed and divided into treatment group and untreated group according to whether oral ibuprofen suspension was used to treat hsPDA or not. Bedside echocardiography and craniocerebral ultrasound were performed immediately after the end of one course of treatment to observe catheter closure and craniocerebral conditions. Results In 30 hsPDA low birth weight infants treated with ibuprofen suspension, the Vs and Vd values of MCA increased after treatment, while RI value decreased(P<0.05). The incidence of severe IVH in 32 patients with Low birth weight hsPDA in untreated group(37.5%) was significantly higher than that in 30 patients with low birth weight hsPDA(13.3%)(P<0.05). Conclusions hsPDA therapy can improve the cerebral blood flow in low birth weight infants and prevent the progression of intraventricular hemorrhage and severe intraventricular hemorrhage to some extent.
