摘 要:目的 探讨常规超声和超声造影对类肿瘤样IgG4相关涎腺炎与涎腺良性肿瘤的鉴别诊断价值。方法 回顾性分析12例类肿瘤样IgG4-RS和30例涎腺良性肿瘤患者的常规超声和超声造影特征。结果 二者在边界、形态、血流信号、结节内液化区、造影增强方式,造影剂分布、增强后边界、增强环、无增强区方面差异有统计学意义(P<0.05)。超声造影定量分析发现峰值强度、流入相和流出相曲线下面积在涎腺良性肿瘤与其周围腺体间差异有统计学意义(P<0.05)。Logistic诊断模型ROC曲线下面积为0.906,诊断准确率为85.71%、灵敏度为93.33%、特异度为78.95%。结论 常规超声及超声造影对类肿瘤样IgG4-RS和涎腺良性肿瘤的鉴别诊断有较高的临床应用价值。Objective To investigate the value of conventional ultrasound and contrast-enhanced ultrasound(CEUS) in the differential diagnosis of tumorlike IgG4-related sialadenitis(IgG4-RS) and benign salivary gland tumors. Methods The conventional ultrasound and CEUS features of 12 patients with tumor-like IgG4-RS and 30 patients with benign salivary gland tumors were analyzed retrospectively. Results There were significant differences in boundary, shape, blood flow, liquefaction area in the nodule, enhancement mode, contrast agent distribution, post enhancement boundary, enhancement ring and non-enhancement area between the two groups(P<0.05). CEUS quantitative analysis showed that there were significant differences in peak enhancement, the area under the curve during wash-in and wash-out between benign salivary gland tumors and their surrounding glands(P<0.05). The area under the ROC curve of logistic diagnostic model was 0.906, with a high diagnostic accuracy of 85.71%, sensitivity of 93.33%, specificity of 78.95%. Conclusions Conventional ultrasound and CEUS have high clinical value in the differential diagnosis of tumor like IgG4-RS and benign salivary gland tumors.
