摘　　要：目的 探讨基于左心声学造影的斑点追踪评估心尖肥厚型心肌病(ApHCM)患者左心室心肌分层纵向应变特征。方法 分层斑点追踪技术回顾性分析46例ApHCM患者与21例正常对照左心声学造影图像,测量整体及各节段分层纵向应变(LS)并分析。结果 病例组左心室分层及各节段LS值均低于对照组,其中混合型ApHCM组最低(P<0.05)。单纯型ApHCM组除增厚的心尖段心肌LS值降低外,厚度正常的中段及基底段心肌LS值亦较对照组降低(P<0.05)。结论 ApHCM患者左心室各层、各节段心肌LS值均显著低于对照组。基于左心声学造影的斑点追踪有望对ApHCM患者诊断及预后评估发挥重要作用。Objective To investigate the characteristics of longitudinal myocardial strain(LS) in patients with apical hypertrophic cardiomyopathy(ApHCM) by the layer-specific speckle tracking based on left ventricle contrast-enhanced echocardiography(CE). Methods The CE images of 46 patients with ApHCM and 21 normal controls were analyzed retrospectively. The global and segmental LS values of layer-specific strain were measured for further analysis. Results The layer-specific global and segmental LS values of the left ventricule in the ApHCM groups were lower than those in the control group, with the mixed ApHCM group as the most significant(all P<0.05). In the pure ApHCM group, the LS values of the myocardium in the normal thickness range were significantly lower than those in the control group(all P<0.05) besides the thickened apical segment. Conclusions The layer-specific and segmental LS values of the left ventricle in the ApHCM patients were significantly lower than those in the control group. The layer-specific speckle tracking technique based on CE is expected to play an important role in the diagnostics and prognostics of the ApHCM patients.
