摘 要:目的 总结小儿原发性大网膜扭转的超声特征及相关临床表现。方法 回顾性分析超声检查并经手术及病理确诊的5例原发性大网膜扭转患儿的术前超声表现和临床特征。结果 5例患儿中术前超声准确诊断4例。超声显示右中下腹部紧贴腹壁的高回声团块,肠间隙少量积液。4例术前超声确诊患儿高回声团块边缘可显示“螺旋征”。5例患儿均以右侧腹痛就诊,有4例体质量超重,所有患儿均采用腹腔镜手术切除病灶。结论 小儿原发性大网膜扭转罕见;加强对该疾病认识、掌握其超声特征后,超声可在术前做出准确诊断。Objective To summarize ultrasound characteristics and clinical manifestations of primary omental torsion in children, aiming to improve the understanding of this disease and the accuracy rate of diagnostic ultrasound. Methods Preoperative characteristics of the ultrasound and clinical manifestations of 5 children with pathologically diagnosed primary omental torsion who were surgically treated in our hospital from July 2020 to October 2021 were retrospectively analyzed. Results 4/5 children were accurately diagnosed as primary omental torsion by preoperative ultrasound. Ultrasound of the 5 children all showed a hyperechoic mass close to the abdominal wall in the right middle and lower abdomen, and a small amount of fluid in the intestinal space. The torsion knot sign at the edge of the hyperechoic mass was examined in 4 children who were accurately diagnosed as primary omental torsion by preoperative ultrasound. All the 5 children presented with right abdominal pain, 4/5 were overweight. Lesions were all removed by laparoscopic surgery. Conclusions Pediatric primary omental torsion is rare. The ultrasound is able to preoperatively diagnose this disease. Improving the understanding of primary omental torsion and mastering its characteristics of ultrasound contribute to the accurate diagnosis of primary omental torsion, thus providing reliable information for surgical resection.
