摘  要:目的 探讨成年人群全血硒水平与血压水平之间的关系。方法 回顾性分析美国国家健康与营养调查(national health and nutrition examination survey,NHANES)数据库2017~2018年4985例体检者的临床资料。通过四分位法将血硒水平划分为Q1~Q4组,比较组间基线资料。依次采用多变量线性回归、限制性立方样条和亚组分析来探索血硒水平与血压关联及剂量-反应关系。结果 组间收缩压、舒张压水平及高血压病史比较,差异均有统计学意义(P<0.05)。调整混杂因素后的回归分析结果显示,与Q1组比较,Q4组舒张压升高了0.82mmHg(95%CI:0.57~1.07mmHg)。限制性立方样条分析显示硒与收缩压呈线性正相关,与舒张压的关系呈S形。亚组分析结果显示,硒与高血压史的交互作用显著。结论 血硒水平与血压水平呈同向递增,且与舒张压水平独立相关。Objective To investigate the association between the level of Blood selenium and blood pressure(BP) in adults.Methods Data of 4985 participants from the National Health and Nutrition Examination Survey(NHANES) from 2017 to 2018 were analyzed.Blood selenium was categorized into quartiles,and the baseline data among the goups were compared.Multivariable linear regression model,restricted cubic splines(RCS) and sub group analysis were used to explore the dose-response relationships between blood selenium concentration and blood pressure.Results There were significant differences in systolic BP and diastolic BP and hypertension among groups(all P<0.05).Comparing with the lowest quartile,the highest quartile diastolic BP increased by 0.82 mmHg(95% CI:0.57-1.07 mmHg) after adjusting for confounding factors.RCS revealed the relationship was S-shaped curve.Sub group analysis revealed a significant interaction between selenium and hypertension.Conclusion Blood selenium concentration changes in the same direction as blood pressure and was independently associated with diastolic BP.
