CD4^(+) CD25^(+) Foxp3^(+)调节性T细胞在原发眼附属器MALT淋巴瘤患者中的表达及临床意义

摘　　要：目的 探讨原发眼附属器MALT淋巴瘤(ocular adnexal MALT,OAML)患者外周血中CD4^(+)CD25^(+)Foxp3^(+)调节性T细胞(regulatory T cell,Treg)的临床意义。方法 回顾性分析笔者医院44例初诊OAML患者外周血Treg细胞比例与临床特征、预后等因素的关系。结果 共入组44例患者,男女性别比为1.75,发病年龄为20~79岁,中位发病年龄为60岁。OAML患者外周血Treg细胞比例为7.63%±3.77%,高于正常对照(6.09%±1.02%),两者比较,差异有统计学意义(P=0.015)。伴有眼外组织受累OAML患者外周血Treg细胞比例高于病变局限于眼附属器患者(P=0.030),骨髓受累患者Treg细胞比例明显高于无骨髓受累患者(P<0.001)。低Treg细胞患者组(n=28)3年无进展生存(progression free survival,PFS)率为92.9%,高Treg细胞患者组(n=16)3年PFS率为66.8%(P=0.155)。结论 Treg细胞在OAML发病过程中可能发挥了免疫逃逸作用,外周血中Treg细胞比例增高提示可能存在眼附属器以外部位的累及。Objective To investigate the clinical significance of peripheral blood CD4^(+)CD25^(+)Foxp3^(+) regulatory T(Treg) cells in primary ocular adnexal MALT lymphoma(OAML).Methods We performed a retrospective analysis of the associations of blood Treg cells with the clinical characteristics and patient survival of 44 patients with a newly diagnosed OAML in Beijing Tongren Hospital.ResultsThe male-to-female ratio of 44 patients was 1.75 and the age of diagnosis was 20-79 years old,the median age of diagnosis was 60 years in this study sample.The proportion of Treg cells in peripheral blood was higher among patients with OAML 7.63%±3.77%,than that of disease-free controls 6.09%±1.02%(P=0.015).The proportion of Treg cells was higher among OAML patients with extraocular involvement than that of patients with lesions limited to ocular adnexa(P=0.030).The proportion was also higher among OAML patients with bone marrow involvement,compared with patients without such involvement(P<0.001).The 3-year PFS rate was 92.9% among patients with low proportion of Treg cells(n=28) and 66.8% among patients with high proportion of Treg cells(n=16),although the difference was not statistically significant(P=0.155).Conclusion Treg cells might play an important role in immune escape during the pathogenesis of OAML,and the increased proportion of Treg cells in peripheral blood might indicate the involvement of extraocular tissue sites.
