任继学从“毒损肾络”论治IgA肾病

摘　　要：任继学教授认为,IgA肾病的病因病机是正气亏虚,感受邪毒,肾脏体用俱损;标实多为感而即发的外感六淫邪毒,如风毒、寒毒、湿毒、热毒等;或者外邪稽留体内伏而未发,日久形成热毒、痰毒、瘀毒等,本虚多责之于肾,与肺、脾关系密切,病机关键是邪毒内蕴,扰动肾络。治疗原则急性期以祛邪为主,缓解期以标本兼治,慢性期以扶正为主兼以通络。任老尤其提出“毒损肾络”理论分期论治IgA肾病,辨治过程中注重未病先防,既病防变。Professor Ren Jixue maintains that the etiology and pathogenesis of IgA nephropathy is the deficiency of vital qi and the exogenous invasion of pathogenic toxins,which leads to the damage of the kidney and its functions.In his view,the excess in manifestations is mostly caused by the external six climatic exopathogens,such as wind toxin,cold toxin,damp toxin,heat toxin,etc.,which may instantly induce the disease as they assault the kidney,or caused by exogenous pathogens,which stay in the body and do not develop into a disease rather,over time,shape heat toxin,phlegm toxin,blood stasis toxin,etc.While the deficiency is mostly rooted in the kidney,yet closely related to the lung and spleen as well,whose key pathogenesis is the internal accumulation of pathogenic toxins,resulting in disturbance to the kidney collaterals.He argues that the treatment principle for IgA nephropathy is to eliminate pathogenic factors in the acute stage,to treat both its manifestations and root causes in its remission stage,while to focus on strengthening the body as well as dredging collaterals in its chronic stage.What counts more is that Professor Ren Jixue puts forward the theory of“toxins damaging the kidney collaterals”and the concept of treating IgA nephropathy by stages.While in the process of syndrome di?erentiation and treatment,he attaches great importance to prevention before its onset and to preventing the established disease from transmission.
