微视野计检查联合OCT评估康柏西普治疗糖尿病性黄斑水肿的效果

摘  要:目的观察MP-3微视野计检查联合OCT评估玻璃体内注射康柏西普治疗糖尿病性黄斑水肿(DME)的效果。方法回顾性病例系列研究。分析郑州大学第一附属医院2020年6月至2021年8月共39例(68只眼)DME的临床资料。按照黄斑区视网膜在OCT特征性病理变化的不同,分为早期组30只眼、进展组38只眼。应用OCT及MP-3微视野计观察注射前后两组患者的视力(logMAR)、黄斑中心区厚度(CMT)、视网膜平均敏感度(MLS)及椭圆体带的变化。结果注射后两组视力(早期组:0.17±0.10,进展组:0.38±0.23)、CMT[早期组:(292.87±28.99)μm,进展组:(355.39±86.94)μm]及MLS[早期组:(22.65±1.81)dB,进展组:(20.03±3.05)dB],与注射前相比差异均具有统计学意义(均P<0.05)。注射前进展组椭圆体带不完整眼的MLS、视力较椭圆体带完整眼更差(t=1.11,2.54;P=0.020,0.028)。结论MP-3微视野计检查联合OCT能评估玻璃体内注射康柏西普治疗DME可以提高视力,改善黄斑水肿,提高MLS及修复椭圆体带。Objective To observe the application of MP-3 and microperimetry utilized and optical coherence tomography(OCT)to evaluate the efficacy of intravitreal injection of conbercept in the treatment of diabetic macular edema(DME).Methods This was a retrospective case series study.The clinical data of 68 eyes of 39 patients with diabetic macular edema from Jun.2020 to Aug.2021 in the First Affiliated Hospital of Zhengzhou University were analyzed.Based on the characteristic pathological changes of macular retina from OCT,all patients were divided into an early stage group(30 eyes)and a progressive group(38 eyes).OCT and MP-3 microperimetry were used to observe the changes of visual acuity(logMAR),central macular thickness(CMT),the retinal mean light sensitivity(MLS)and ellipsoidal zone.Results There were statistically significant differences in postoperative visual acuity(early stage group:0.17±0.10,progressive group:0.38±1.81),CMT[early stage group:(292.87±28.99)μm,progressive group:(355.39±86.94)μm]and MLS[early stage group:(22.65±1.81)dB,progressive group:(20.03±3.05)dB]in the two groups before and after injection(all P<0.05).In the progressive group,the MLS and visual acuity of ellipsoidal zone incomplete eyes were worse than those of ellipsoidal zone intact eyes(t=1.11,2.54;P=0.020,0.282).Conclusion Application of MP-3 microperimetry combined with OCT can evaluate the therapeutic efficacy safely and effectively.Intravitreal injection of conbercept can improve visual acuity,reduce macular edema,increase MLS and repair ellipsoidal zone in the treatment for DME.
