摘　　要：目的 创新性组建重症快速拓展团队(CROT)这一院内快速反应系统,并评价其对住院病情变化患者结局的影响。方法 建立与运行CROT体系,包括预警标准、启动方法、团队组成、设备配置以及高效的运行机制。回顾性前后对照探讨CROT运行前后3年所有普通病区住院患者非计划入住ICU率、非计划入住ICU间隔时间、ICU入住时间及死亡率、心跳呼吸骤停发生率、抢救成功率以及住院患者死亡率。结果 CROT运行后,普通病区住院患者非计划入住ICU率及入住ICU后死亡率下降,非计划入住ICU间隔时间及ICU入住时间缩短,心跳呼吸骤停发生率下降且抢救成功率明显提高,差异均有统计学意义(P<0.05)。但在住院患者整体死亡率上差异无统计学意义(P>0.05)。结论 CROT体系的建立有助于早期识别及干预ICU之外的危重症患者,改善住院病情变化患者结局,最大限度保证普通病房患者医疗安全,值得大力推广。Objective To innovatively set up the Critical Rapid Outreach Team(CROT),an in-hospital Rapid Response System,and evaluate its impact on the outcomes of inpatients whose conditions have changed. Methods It designed and operated the CROT system,including early warning standards,start-up methods,team composition, equipment configuration and efficient operation mechanism. The unplanned ICU admission rate, unplanned ICU admission interval,ICU hospitalization time and mortality rate,cardiorespiratory arrest incidence,rescue success rate and mortality of hospitalized patients of all hospitalized patients in general wards for three years before and after CROT functioning were investigated by a retrospective before-after controlled method. Results After the functioning of CROT, the unplanned ICU admission rate and mortality rate of inpatients in the general ward decreased,the unplanned ICU admission interval and the ICU hospitalization time were shortened,the mortality rate after admission to ICU decreased,the cardiorespiratory arrest incidence decreased,and the rescue success rate was significantly increased,all the differences were statistically significant(P<0.05). However,there was no significant difference in the overall mortality of hospitalized patients(P>0.05). Conclusion The establishment of the CROT system is helpful for the early identification and intervention of critical patients who are not admitted to the ICU. It can improve the outcome of patients whose conditions have changed,and ensure the medical safety of patients in general wards to the greatest extent and the model is worthy of vigorous promotion.
