摘  要:目的比较3D打印个性化支具与铰链式支具治疗内侧副韧带损伤的临床疗效。方法前瞻性分析2021年1月—2021年7月于河北医科大学第一医院骨科就诊的运动性内侧副韧带损伤(核磁共振显示Ⅰ度或Ⅱ度损伤)患者共60例,男性29例,女性31例;年龄13~58岁,平均35.7岁。采用随机数字表法分为两组:3D打印支具组(n=30)和铰链式支具组(n=30)。铰链式支具组患膝佩戴同一品牌膝关节铰链式支具,3D打印支具组佩戴3D打印个性化支具。治疗周期为2周,记录并比较两组治疗前与治疗2周后步态分析仪测量下的膝关节最大外翻角度、患肢步态周期、患肢支撑期和行走时屈伸范围,Ligs关节韧带数字检查仪测量下80、120、150N外翻应力下胫骨相对位移距离,国际膝关节文献委员会(IKDC)评分和满意度。结果治疗后,两组膝关节最大外翻角度、行走时屈伸范围、患肢步态周期、患肢支撑期比较差异无统计学意义(P>0.05)。两组组内治疗前后比较,行走时屈伸范围差异有统计学意义(P<0.05)。两组在80、120、150N外翻应力下3D打印支具组与铰链式支具组患者胫骨相对位移距离差异均无统计学意义(P>0.05),两组IKDC评分比较差异无统计学意义(P>0.05)。3D打印支具组患者满意率较铰链式支具组升高(P<0.05)。结论3D打印个性化支具与铰链式支具治疗内侧副韧带损伤的临床效果相当,但3D打印个性化支具佩戴更加舒适便捷,患者对于佩戴3D个性化打印支具有较高的满意度。Objective To compare the clinical efficacy of 3D printed personalized brace and hinged brace in the treatment of medial collateral ligament injury.Methods A total of 60 patients with athletic medial collateral ligament injury(degreeⅠorⅡinjury revealed by MRI)who were treated in the Department of Orthopaedics of the First Hospital of Hebei Medical University from Jan.2021 to Jul.2021 were selected and prospectively analyzed.There were 29 males,and 31 females;their age ranged from 13 to 58 years,with an average of 35.7 years.They were randomly divided into two groups:3D printing brace group(n=30)and hinge brace group(n=30).The patients in the hinge brace group wore knee hinge brace of the same brand,and the 3D printing brace group wore the 3D printing personalized brace.The treatment period was 2 weeks,the maximum valgus angle of the knee joint,the gait cycle of the affected limb,the support period of the affected limb,the range of flexion and extension during walking,the relative displacement distance of the tibia under the valgus stress of 80N,120N and 150N measured by the Ligs joint ligament digital tester,the score of the International Knee Documentation Committee(IKDC)and the satisfaction of patients were recorded and compared between the two groups before and after 2 weeks of treatment.Results After 2 weeks of treatment,there was no significant difference in the maximum valgus angle of the knee joint,the range of flexion and extension during walking,the gait cycle of the affected limb and the support period of the affected limb between the two groups(all P>0.05).There was significant difference in the range of flexion and extension during walking between the two groups before and after treatment(all P<0.05).There was no significant difference in tibial relative displacement under 80N,120N and 150N valgus stress between the two groups(all P>0.05).There was no significant difference in IKDC score between the two groups(P>0.05).The satisfaction rate of 3D printing brace group was higher than that of hinge br
