摘 要:目的探索冠心病患者外周血Dickkopf相关蛋白1(DKK-1)、骨桥蛋白水平与冠状动脉病变程度的关系及其对冠心病病变程度的诊断效能。方法回顾性选取2018年2月至2020年10月期间绵阳市中心医院收治的61名健康体检者(对照组)、135例冠心病患者(观察组)作为研究对象。观察组患者根据造影结果显示的冠状动脉病变累及血管支数分为3组,35例为三支病变组、45例为双支病变组、55例为单支病变组,均进行DKK-1、骨桥蛋白检测后,比较各组DKK-1、骨桥蛋白水平,分析DKK-1、骨桥蛋白与Gensini积分、冠状动脉病变程度相关性,再经受试者工作特征(ROC)曲线分析DKK-1、骨桥蛋白单独及两项联合检测对冠状动脉病变程度的诊断效能。结果观察组患者DKK-1、骨桥蛋白水平以及Gensini积分均明显高于对照组,差异均有统计学意义(P<0.05);三支病变组DKK-1、骨桥蛋白水平以及Gensini积分均明显高于双支组和单支病变组,差异均有统计学意义(P<0.05)。经Pearson法分析,Gensini积分与DKK-1、骨桥蛋白呈正相关性;经Spearman法分析,冠状动脉病变程度与DKK-1、骨桥蛋白、Gensini积分呈正相关性;经ROC曲线分析,DKK-1、骨桥蛋白单独及两项联合诊断冠状动脉三支病变的曲线下面积分别为0.865、0.817、0.890。结论外周血DKK-1、骨桥蛋白水平与冠心病发生、发展密切相关,检测DKK-1、骨桥蛋白水平可作为诊断冠状动脉病变程度的重要指标。Objective To explore the relationship between Dickkopf-related protein 1(DKK-1)and osteopontin levels in peripheral blood of patients with coronary heart disease and the degree of coronary artery disease and its diagnostic efficacy.Methods A total of 61 healthy people(control group)and 135 patients with coronary heart disease(observation group)admitted to Mianyang Central Hospital from February 2018 to October 2020 were retrospectively selected as the research objects.According to the number of coronary artery lesions involved in the observation group,the patients were divided into three groups,35 were three vessel lesions group,45 were double vessel lesions group and 55 were single vessel lesions group.After DKK-1 and osteopontin detection,the levels of DKK-1 and osteopontin in each group were compared,the correlation between DKK-1 and osteopontin and Gensini score and the degree of coronary artery disease were analyzed,and then the diagnostic efficacy of DKK-1 and osteopontin alone and jointly by receiver operating characteristics(ROC)curve were analyzed.Results The levels of DKK-1,osteopontin and Gensini score in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The levels of DKK-1,osteopontin and Gensini score in the three-vessel disease group were higher than those in the double-vessel disease group and the single-vessel disease group,the differences were statistically significant(P<0.05).According to the analysis of Pearson method,Gensini score was positively correlated with DKK-1 and osteopontin;by Spearman method,the degree of coronary artery disease was positively correlated with DKK-1,osteopontin,Gensini score;after ROC curve analysis,the AUC of DKK-1,osteopontin and the two combined predictions of three-vessel coronary artery disease are 0.865,0.817,0.890,respectively.Conclusion Peripheral blood DKK-1 and osteopontin levels are closely related to the occurrence and development of coronary heart disease,and can be used as important i
