摘　　要：目的 探讨老年透析患者miR-21与外周血T淋巴细胞亚群之间的相关性。方法 将2020年1月至2021年1月北京友谊医院老年医学科收治的老年维持性血液透析患者28例纳入观察组,47例老年健康体检者纳入对照组,检测对照组入组时及观察组透析前外周血T淋巴细胞亚群及miR-21、miR-23a、miR-150、miR-100、miR-128、miR-211表达水平,以及透析前后miR-21与CD4、Treg细胞百分率的相关性。结果 与对照组相比,观察组透析前收缩压、尿素氮、中性粒细胞百分比、白介素-6水平明显增高,血红蛋白、谷丙转氨酶、白蛋白、估算的肾小球滤过率明显偏低(P<0.001),CD4和Treg细胞百分率均显著增高(P<0.001),miR-21-5p相对表达量有所增高(P<0.05)。透析后观察组估算的肾小球滤过率、尿素氮、血磷、血钾水平明显下降(P<0.001),且透析后miR-21相对表达量和Treg细胞的比率均增高(P<0.05), Pearson相关分析显示观察组在透析前后的miR-21相对表达量和Treg细胞比率呈正相关(P<0.05)。结论 老年透析患者存在微炎性反应状态,透析前后血清miR-21表达量与Treg细胞比率呈正相关。Objective To investigate the correlation between miR-21 and peripheral blood lymphocyte subsets in elderly dialysis patients. Methods Twenty-eight elderly maintenance hemodialysis patients were enrolled as the observation group from the Department of Geriatrics of Beijing Friendship Hospital from January 2020 to January 2021. Forty-seven elderly healthy check-up people were included in the control group. The percentage of T lymphocyte subsets and the expression levels of miR-21, miR-23 a, miR-150, miR-100;miR-128 and miR-211 were examined in control group and observation group. The percentage of CD4and Treg cells and the expression levels of miR-21 were detected before and after dialysis. Results The levels of systolic blood pressure, urea nitrogen, neutrophile percentage and interleukin-6 in patients with the observation group were significantly higher than that of the control group(P<0.001). The levels of estimated glomerular filtration rate, hemoglobin, glutamic-pyruvate transaminase, albumin were significantly lower in the observation group(P<0.001), and the percentages of CD4and Treg cells were significantly increased(P<0.001). The relative expression of miR-21-5 p was increased in the observation group(P<0.05). Pearson correlation analysis showed that the relative expression of miR-21 was positively correlated with the ratio of Treg cells in the observation group before and after dialysis(P<0.05). ConclusionsThere are micro-inflammation in elderly patients undergoing dialysis and the expression level of miR-21 in serum is positively correlated with the ratio of Treg cells before and after dialysis.
