老年冠心病患者血浆脂质运载蛋白2表达水平及与侧支循环形成的关系

摘　　要：目的研究老年冠心病患者血浆脂质运载蛋白2(LCN2)表达水平及与侧支循环形成的相关性。方法采用回顾性分析方法,以2020年月1月至2021年6月入院的100例老年冠心病患者为研究对象,将其设为研究组,同时选取同一时期入院的50例体检健康者设为对照组。研究组参考侧支循环形成状况,经Rentrop-Cohen分级划分为0-1级(不良组,52例)与2-3级(良好组,48例)。两组均测定血浆LCN2及基质金属蛋白酶9(MMP-9)水平。采用相关性分析及多元Logistic回归分析法分析老年冠心病患者良好侧支循环形成的影响因素,采用受试者工作特征(ROC)曲线分析血浆LCN2水平对冠状动脉侧支形成良好的诊断效能。结果研究组患者血浆LCN2、MMP-9水平为(37.88±6.46)ng/L、(3.47±0.62)mg/L,均明显高于对照组[(21.18±3.55)ng/L、(1.49±0.41)mg/L],差异有统计学意义(P<0.05)。良好组患者血浆LCN2、MMP-9水平为(41.82±5.93)ng/L、(3.83±0.64)mg/L,均明显高于不良组[(34.27±4.51)ng/L、(3.13±0.43)mg/L],差异均有统计学意义(P<0.05)。研究组患者血浆LCN2与MMP-9水平呈正相关(r=0.611,P<0.01)。多元Logistic回归分析显示,老年冠心病患者良好侧支循环形成的影响因素为血浆LCN2(OR=1.227,P<0.01)、MMP-9(OR=1.001,P=0.026)。ROC曲线分析,以37.88 ng/L为最佳临界值时,血浆LCN2水平在冠状动脉侧支形成良好的诊断中的曲线下面积为0.84,特异度为80.42%,敏感度为83.71%。结论老年冠心病患者血浆LCN2表达水平上升,与MMP-2密切相关,血浆LCN2水平对老年冠心病患者侧支循环形成有较高的诊断价值。Objective To investigate the expression level of plasma lipid carrier protein 2(LCN2)in elderly patients with coronary heart disease and its correlation with collateral circulation.Methods Using a retrospective analysis method,100 elderly patients with coronary heart disease who were admitted to Qinzhou First People's Hospital from January 2020 to June 2021 were selected as the research group,and 50 healthy people who were admitted to the hospital during the same period were selected as the control group.The research group was divided into grade 0-1(poor group,52 cases)and grade 2-3(good group,48 cases)by Rentrop-Cohen classification with reference to the formation of collateral circulation.Plasma LCN2 and matrix metalloproteinase 9(MMP-9)levels were measured in both groups.Correlation analysis and multivariate logistic regression analysis were used to analyze the influencing factors of good collateral circulation in elderly patients with coronary heart disease.Results The levels of plasma LCN2 and MMP-9 in the study group were(37.88±6.46)ng/L,(3.47±0.62)mg/L,which were significantly higher than those in the control group[(21.18±3.55)ng/L,(1.49±0.41)mg/L],and the differences were statistically significant(P<0.05).The levels of plasma LCN2 and MMP-9 in the good group were(41.82±5.93)ng/L,(3.83±0.64)mg/L,which significantly higher than those in the poor group[(34.27±4.51)ng/L,(3.13±0.43)mg/L],and the differences were statistically significant(P<0.05).There was a positive correlation between plasma LCN2 and MMP-9 levels in the study group(r=0.611,P<0.01).Multivariate logistic regression analysis showed that the influencing factors of good collateral circulation in elderly patients with coronary heart disease were plasma LCN2(OR=1.227,P<0.01)and MMP-9(OR=1.001,P=0.026).According to ROC curve analysis,when 37.88 ng/L was the optimal cutoff value,the area under the curve of plasma LCN2 level in the diagnosis of good coronary collateral formation was 0.84,the specificity was 80.42%,and the sensitivity was 83.71%
