右美托咪定复合依托咪酯全凭静脉麻醉在腹腔镜胆囊切除术中的麻醉效果及安全性分析

摘　　要：目的:分析右美托咪定联合依托咪酯应用于腹腔镜胆囊切除术的麻醉效果。方法:选取2018年7月-2021年2月成都市新津区中医医院、石河子市人民医院美国麻醉医师协会(ASA)分级Ⅰ、Ⅱ级择期进行腹腔镜胆囊切除术的100例患者,随机分成对照组(47例)与观察组(53例),对照组采用丙泊酚复合依托醚酯麻醉;观察组采用右美托咪定复合依托咪酯全凭静脉麻醉。观察两组麻醉前后血压(BP)与心率(HR)变化,并对比两组术后恢复情况及拔管时间。结果:麻醉前后,两组BP、HR及SpO水平、不良反应发生率比较,差异均无统计学意义(P>0.05);对照组呼吸恢复、苏醒时间及拔管时间均较观察组晚,苏醒期躁动发生率与VAS评分均较观察组更高(P<0.05)。结论:右美托咪定联合依托咪酯应用于腹腔镜胆囊切除术的麻醉效果较好,能有效改善术后患者苏醒质量,具有较高安全性。Objective:To analyze the anesthesia effect of Dexmedetomidine combined with Etomidate in laparoscopic cholecystectomy.Method:From July 2018 to February 2021,a total of 100 patients who underwent laparoscopic cholecystectomy were randomly selected from Traditional Chinese Medicine Hospital of Xinjin District,Chengdu and Shihezi People’s Hospital,which were gradeⅠ,Ⅱclassified by the American Association of Anesthesiologists(ASA).The patients were randomly divided into control group(47 cases)and observation group(53 cases).The control group was anesthetized with Propofol combined with Etomidate;the observation group was anesthetized with Dexmedetomidine combined with Etomidate for total intravenous anesthesia.The changes in blood pressure(BP)and heart rate(HR)of the two groups before and after anesthesia were observed,and the postoperative recovery and extubation time of the two groups were compared.Result:Before and after anesthesia,the levels of BP,HR and SpO,and the incidence of adverse reactions between the two groups were compared,and there were no significant differences(P>0.05);the breathing recovery,wake-up time,and extubation time in the control group were later than those in the observation group,and the incidence of restlessness in recovery period and VAS scores in the control group were higher than those in the observation group(P<0.05).Conclusion:Dexmedetomidine combined with Etomidate for laparoscopic cholecystectomy has a better anesthetic effect,which can effectively improve the quality of postoperative patients’recovery,and has high safety.
