一次性肛周脓肿根治术治疗肛周脓肿的效果及对肛门功能、创口愈合的影响

摘　　要：目的:探讨肛周脓肿患者采用一次性肛周脓肿根治术治疗的效果及对肛门功能、创口愈合的影响。方法:选取2020年5月-2021年6月绵阳市中心医院收治的共计92例肛周脓肿患者作为研究对象,按照随机摸球法分成观察组(46例)与对照组(46例),对照组实施传统分期手术,观察组实施一次性肛周脓肿根治术。比较两组肛门功能、术后疼痛程度、创口愈合情况及生活质量。结果:观察组与对照组相比,肛门功能正常患者占比明显更高(P<0.05),肛门部分失禁、严重失禁患者占比明显更低(P<0.05);术后3 d,观察组与对照组相比术后疼痛0级患者占比明显更高(P<0.05),2级、3级患者占比明显更低(P<0.05);观察组与对照组相比创口愈合时间、创口腐肉脱落时间明显更早(P<0.05),创口缩小率明显更高(P<0.05);两组术后1个月简明健康调查问卷(SF-36)评分均有明显提高(P<0.05),观察组与对照组相比评分明显更高(P<0.05)。结论:肛周脓肿患者给予一次性肛周脓肿根治术,能够改善患者肛门功能,缓解术后疼痛程度,改善创口愈合情况,提高患者生活质量。Objective:To investigate the effect of one-time radical operation for perianal abscess in patients with perianal abscess and its effect on anal function and wound healing.Method:A total of 92 patients with perianal abscesses who were treated in Mianyang Central Hospital from May 2020 to June 2021 were selected as the research objects and divided into an observation group(46 cases)and a control group(46 cases)according to the random touch ball method.Traditional staged surgery was performed in the control group,and the observation group underwent one-time perianal abscess radical resection.The anal function,postoperative pain degree,wound healing and quality of life were compared between the two groups.Result:Compared with the control group,the proportion of patients with anal normal function in the observation group was significantly higher(P<0.05),and the proportion of patients with partial incontinence and severe incontinence were significantly lower(P<0.05).Three days after operation,compared with the control group,the proportion of grade 0 in the observation group was higher(P<0.05),grades 2 and 3 were significantly lower(P<0.05).Compared with the control group,the observation group had significantly earlier wound healing time and wound slough shedding time(P<0.05),the wound reduction rate was significantly higher(P<0.05).One month after operation,the postoperative simple health questionnaire(SF-36)scores in both groups were significantly improved(P<0.05),and the scores in the observation group were significantly higher than those in the control group(P<0.05).Conclusion:One-time perianal abscess radical resection for patients with perianal abscess can improve patients’anal function,relieve postoperative pain,improve wound healing,and improve patients’quality of life.
