穴位贴敷联合小针刀对老年膝关节骨性关节炎患者骨代谢指标和血液流变学指标的影响

摘　　要：目的:研究穴位贴敷联合小针刀治疗老年膝关节骨性关节炎(KOA)的临床效果。方法:选择2018年8月-2020年8月秦皇岛市中医医院收治的76例KOA患者作为研究对象,采用随机数字表法分为试验组(n=38)和对照组(n=38)。对照组采用基础治疗,试验组同时采用自制穴位贴膏贴敷治疗+小针刀治疗。比较两组治疗效果、骨代谢指标、血液流变学指标及生活质量评分。结果:试验组治疗有效率高于对照组,差异有统计学意义(P<0.05);试验组治疗后25-OH-D、BGP水平高于对照组,差异有统计学意义(P<0.05)。试验组治疗后纤维蛋白原、红细胞沉降率、红细胞聚集指数低于对照组,差异有统计学意义(P<0.05);治疗1个月后、随访6个月后试验组关节炎生活质量测量量表2-短卷(AIMS2-SF)评分高于对照组,差异有统计学意义(P<0.05)。结论:穴位贴敷联合小针刀能提升老年KOA患者治疗效果,改善患者骨代谢指标及血液流变学指标水平,对改善患者生活质量亦有积极作用,值得临床推广。Objective:To study the clinical effect of acupoint application combined with small needle knife in the treatment of elderly patients with knee osteoarthritis(KOA).Method:A total of 76 patients with KOA treated in Qinhuangdao Traditional Chinese Medicine Hospital from August 2018 to August 2020 were selected as the research objects and divided into experimental group(n=38)and control group(n=38)by random number table method.The control group was treated with basic treatment,and the experimental group was simultaneously treated with self-made acupoint plaster+small needle knife.The treatment effect,bone metabolism index,hemorheology index and quality of life score were compared between the two groups.Result:The effective rate of the experimental group was higher than that of the control group(P<0.05).The 25-OH-Dand BGP levels in experimental group were higher than those in control group after treatment,and the differences were statistically significant(P<0.05).After treatment,fibrinogen,erythrocyte sedimentation rate and erythrocyte aggregation index in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 1 month treatment and 6 months follow-up,the score of arthritis quality of life scale 2-short volume(AIMS2-SF)in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Acupoint application combined with small needle knife can improve the treatment effect of elderly patients with KOA,improve the level of bone metabolism indexes and hemorheology indexes,and also play a positive role in improving the quality of life of patients,which is worthy of clinical promotion.
