摘　　要：目的:分析左炔诺孕酮宫内缓释节育系统(LNG-IUS)联合促性腺激素释放激素激动剂(GnRH-a)治疗子宫腺肌病的临床效果,为今后临床应用提供参考依据。方法:选取张掖市甘州区人民医院2018年12月-2021年9月收治的子宫腺肌病患者70例,按照数字表随机法分成两组,对照组(n=35)应用GnRH-a治疗,观察组(n=35)在此基础上联合LNG-IUS治疗,比较两组用药期间不良反应情况、痛经程度、子宫体积、月经失血情况。结果:观察组总不良反应发生率(17.14%)与对照组(14.29%)比较,差异无统计学意义(P>0.05)。两组治疗后视觉模拟评分(VAS)、月经失血图(PBAC)评分均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。两组治疗后子宫体积均小于治疗前,且观察组小于对照组,差异均有统计学意义(P<0.05)。结论:GnRH-a联合LNG-IUS治疗子宫腺肌病,安全性较高,可减轻痛经程度,降低PBAC评分,缩小子宫体积,治疗效果优于单独用药。Objective:To analyze the clinical effect of Levonorgestrel-releasing intrauterine system (LNG-IUS) combined with gonadotropin-releasing hormone agonist (GnRH-a) in the treatment of adenomyosis,and to provide reference for future clinical application.Method:A total of 70 cases of adenomyosis patients admitted to Zhangye Ganzhou District People’s Hospital from December 2018 to September 2021 were selected and randomly divided into two groups according to the number table.The control group (n=35) was treated with GnRH-a,and the observation group (n=35) was treated with LNG-IUS on the basis of the control group.The adverse reactions,dysmenorrhea,uterine volume and menstrual blood loss were compared between the two groups.Result:There was no significant difference in the incidence of total adverse reactions between the observation group (17.14%) and the control group (14.29%) (P>0.05).The visual analog scale (VAS) and pictorial blood loss assessment chart (PBAC) scores of the two groups after treatment were lower than those before treatment,and the scores in the observation group were lower than those of the control group,the differences were statistically significant (P<0.05).The uterine volumes after treatment were smaller in both groups than those before treatment,which in the observation group was smaller than that in the control group,the differences were statistically significant (P<0.05).Conclusion:GnRH-a combined with LNG-IUS has high safety in the treatment of adenomyosis,which can relieve dysmenorrhea,reduce PBAC score and reduce uterine volume.The therapeutic effect is superior to that of single drug.
