Prolift全盆底重建术联合雌激素治疗重度盆腔器官脱垂患者的效果

摘　　要：目的:探讨Prolift全盆底重建术联合雌激素治疗重度盆腔器官脱垂(pelvic organ prolapse,POP)患者的效果。方法:回顾性分析三明市第一医院2019年3月-2021年3月收治的130例重度POP患者。按治疗方案将其分为对照组(n=64)和观察组(n=66)。对照组予以常规传统的阴式子宫切除术联合阴道前后壁修复治疗,观察组予以Prolift全盆底重建术联合雌激素治疗。比较两组术前及术后6个月POP-Q指示点评分、盆底功能恢复效果及脱垂复发率。结果:术后6个月,观察组Aa、Ba、C、Bp小于对照组(P<0.05)。术后6个月,观察组盆底功能障碍疾病症状问卷简表(PFDI-20)评分低于对照组(P<0.05)。观察组脱垂复发率低于对照组(χ^(2)=4.431,P<0.05)。结论:对于重度POP患者使用Prolift全盆底重建术联合雌激素治疗可有效恢复患者阴道正常解剖位置,改善盆底功能,降低脱垂复发风险。Objective:To investigate the effect of Prolift total pelvic floor reconstruction surgery combined with estrogen in the treatment of patients with severe pelvic organ prolapse(POP).Method:A retrospective analysis was performed on 130 patients with severe POP treated in First Hospital of Sanming from March 2019 to March 2021.They were divided into control group(n=64)and observation group(n=66)according to treatment plan.The control group was given the conventional vaginal hysterectomy combined with anterior and posterior vaginal wall repair treatment,and the observation group was given the Prolift total pelvic floor reconstruction surgery combined with estrogen treatment.POP-Q indicator point score,pelvic floor function recovery effect before and 6 months after surgery and prolapse recurrence rate were compared between the two groups.Result:Six months after operation,Aa,Ba,C,Bp in the observation group were smaller than those in the control group(P<0.05).Six months after surgery,the score of short form of pelvic floor dysfunction disease symptom questionnaire(PFDI-20)in the observation group was lower than that in the control group(P<0.05).The prolapse recurrence rate in the observation group was lower than that in the control group(χ^(2)=4.431,P<0.05).Conclusion:For patients with severe POP,Prolift total pelvic floor reconstruction surgery combined with estrogen treatment can effectively restore the normal anatomical position of the vagina,improve pelvic floor function and reduce the risk of prolapse recurrence.
