内外固定分期序贯与切开复位内固定治疗高能量损伤Pilon骨折的临床对比研究

摘　　要：目的:探究与分析内外固定分期序贯与切开复位内固定治疗高能量损伤Pilon骨折的临床效果。方法:回顾性分析山东国欣颐养集团枣庄中心医院自2018年5月-2021年1月收治的Pilon骨折患者98例的临床资料,按照不同手术治疗方法分为序贯组(n=47)与内固定组(n=51),序贯组给予内外固定分期序贯治疗,内固定组给予切开复位内固定治疗,对比两组患者临床疗效、骨折愈合时间、复位情况、治疗前后踝关节Kofoed评分及Baird评分,同时观察术后并发症。结果:序贯组与内固定组相比临床总有效率较高,复位情况更好,差异有统计学意义(P<0.05)。序贯组术后3、6个月Kofoed评分及Baird评分高于内固定组,差异有统计学意义(P<0.05)。序贯组与内固定组的术后并发症发生率相比差异无统计学意义(P>0.05)。结论:内外固定分期序贯与切开复位内固定治疗高能量损伤Pilon骨折均可获得较好的临床效果,改善踝关节功能,促进骨折愈合,且相比而言前者的综合效果更好。Objective:To explore and analyze the clinical effect of sequential internal and external fixation and open reduction internal fixation in the treatment of high-energy injury Pilon fracture.Method:The clinical data of 98 Pilon fracture patients admitted to Shandong Guoxin Health Care Group Zaozhuang Central Hospital from May 2018 to January 2021 were retrospectively analyzed.According to different surgical treatment methods,they were divided into sequential group(n=47)and internal fixation group(n=51).The sequential group received sequential treatment with internal and external fixation,and the internal fixation group received open reduction internal fixation.The clinical efficacy,fracture healing time,reduction,ankle Kofoed score and Baird score before and after treatment were compared between the two groups,and postoperative complications were observed.Result:Compared with the internal fixation group,the sequential group had higher total clinical effective rate,better reduction,and the differences were statistically significant(P<0.05).The Kofoed score and Baird score in the sequential group were higher than those in the internal fixation group at 3 and 6 months after surgery,respectively,and the differences were statistically significant(P<0.05).There was no statistical significance in the incidence of postoperative complications between the sequential group and the internal fixation group(P>0.05).Conclusion:Sequential internal and external fixation and open reduction internal fixation in the treatment of high-energy injury Pilon fracture can achieve better clinical effect,improve ankle joint function,promote fracture healing,and the former has better comprehensive effect.
