沙库巴曲缬沙坦治疗高血压合并心力衰竭对患者心功能及血浆脑钠肽水平的影响

摘　　要：目的 分析沙库巴曲缬沙坦对高血压合并心力衰竭的治疗效果。方法 选取福建省漳州市第二医院心血管内科于2020年1月至2021年1月入院治疗的高血压合并心力衰竭患者83例。采用随机数字表法分组,观察组42例实行沙库巴曲缬沙坦+常规治疗,对照组41例实行常规治疗,比较两组心功能改善情况、血浆脑钠肽(BNP)变化和总有效率。结果 治疗前,两组患者左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、二尖瓣口舒张早期血流速度/二尖瓣舒张晚期血流速度(E/A)指标比较,差异无统计学意义(P> 0.05)。治疗6个月后,观察组患者LVEF、LVESD、LVEDD、E/A指标均优于对照组,差异有统计学意义(P <0.05)。治疗前,两组患者的BNP水平比较,差异无统计学意义(P> 0.05)。治疗6个月后,观察组患者的BNP水平低于对照组,差异有统计学意义(P <0.05)。观察组患者的总有效率高于对照组,差异有统计学意义(P <0.05)。结论 沙库巴曲缬沙坦可有效改善高血压合并心力衰竭患者心功能及BNP水平,提高治疗效果。Objective To analyze the therapeutic efficacy of sacubitril valsartan on hypertension complicated with heart failure. Methods A total of 83 patients with hypertension complicated with heart failure admitted to and treated in the Department of Cardiovascular Internal Medicine of the Second Hospital of Zhangzhou City in Fujian Province from January 2020 to January 2021 were selected. They were divided into the observation group(n=42) and the control group(n=41) according to the random number table method. The observation group was treated with sacubitril valsartan sodium tablets combined with conventional therapy, while the control group was treated with conventional therapy. The improvement of cardiac function, the changes in plasma brain natriuretic peptide(BNP) and the overall response rate were compared between the two groups. Results Before treatment,there were no statistically significant contrast differences in the indices of left ventricular ejection fraction(LVEF), left ventricular end-systolic diameter(LVESD), left ventricular end-diastolic diameter(LVEDD),and early diastolic blood flow velocity of mitral valve/late diastolic blood flow velocity of mitral valve(E/A)between the two groups(P > 0.05). After 6 months of treatment, the indices of LVEF, LVESD, LVEDD and E/A in patients of the observation group were all better than those in the control group, with statistically significant differences(P < 0.05). Before treatment, there was no statistically significant difference in BNP levels between the two groups of patients(P > 0.05). After 6 months of treatment, the BNP level of patients in the observation group was lower than that in the control group, with statistically significant difference(P < 0.05). The overall response rate of patients in the observation group was higher than that in the control group, with statistically significant difference(P < 0.05).
