基于TUGT表现的健康成年人群(45~74岁)日常活动能力的定量分级

摘　　要：目的:探讨基于计时“起立-行走”测试(TUGT)表现的健康成年人群(45~74岁)日常活动能力的定量分级标准,以期制定一个方便、快捷的健康成年人群日常活动能力水平的定量筛选方法。方法:用SPSS22.0将健康成年受试者(45~74岁)的TUGT成绩以由高到低的顺序10等分,将前10%定义为高于正常水平,中间80%定义为正常水平,后10%定义为低于正常水平。使用ridit分析检验TUGT表现分级的特异性,使用有序逻辑回归分析检验TUGT表现分级的一致率,并使用多重线性逐步回归分析构建影响TUGT表现的最优自变量组合模型。结果:中年人群(45~64岁)TUGT成绩分级的临界值为5.84s和8.6s,当TUGT成绩> 8.6s时低于正常水平;年轻老年人群(65~74岁)TUGT成绩分级的临界值为6.6s和10.1s,当TUGT成绩> 10.1s时低于正常水平。Ridit分析结果显示该分级标准具有特异性。有序逻辑回归分析结果显示,中年人群TUGT表现的平均水平与10m计时行走测试、5次坐立测试、功能性前伸测试和闭眼单腿站立测试表现以及髋关节、膝关节、踝关节和生活质量(SF-36)的主观感觉量表表现的平均水平的一致率均高于79%,而年轻老年人群的一致率均高于86%。多重线性逐步回归分析结果显示影响中年人群TUGT表现的主要因素依次为行走、坐立和功能性前伸能力,而影响年轻老年人群TUGT表现的因素主要为行走和坐立能力。结论:TUGT表现分级可较好地反映健康成年人群(45~74岁)日常活动能力的客观功能状态和主观功能感觉。本研究结果补充和完善了TUGT表现<13.3s区间的评价,同时也为提高TUGT表现提供了思路和方法。The paper aims was to develop a convenient and fast quantitative screening method to identify activities levels of daily living in healthy adults(45~74 years old) based on TUGT performance.TUGT scores of healthy adult subjects(45~74 years old) were divided into 10 grades in order of high to low by SPSS 22.0.The first 10% was defined as above the normal level,the middle 80% as the normal level,and the last 10% as below the normal level.The specificity of TUGT performance grades was tested by ridit analyses,and the consistency was tested by ordinal logistic regression.Multiple stepwise linear regression analyses were conducted to construct models for identifying independent contributors to TUGT performance.The study showed that the cut-off values of TUGT scores in middle-aged population(45~64 years old) were 5.84 s and 8.6 s,which was below the normal level when the TUGT score was more than 8.6 s;and the cut-off values of TUGT scores in younger aged population(65~74 years old) were 6.6 s and 10.1 s,which was below the normal level when the TUGT score was more than 10.1 s.Ridit analyses results showed that the grading standard was specific,and ordinal logistic regression results showed that the concordance rates were higher than 79% between the average level of TUGT performance and the average levels of 10 m TWT,5-STST,FRT,OLS-ECT,HAGOS,IKDC,FAAM or SF-36 in middle-aged population,while the concordance rates in younger aged population were higher than86%.The results of multiple stepwise linear regression showed that the factors affecting TUGT performance in middle-aged population were walking,sit-to-stand and functional reach abilities,while the factors in younger aged population were mainly walking and sit-to-stand abilities.It is concluded that grades of TUGT performance could better reflect the objective functional conditions and the subjective functional sensations of daily living activities.The results of the study has complemented and improved the evaluation of TUGT performance in the region with TUGT score le
