质子泵抑制剂用药规则设置在我院门诊处方前置审核中的实践

摘　　要：目的 探讨质子泵抑制剂用药规则设置在医院门诊处方前置审核中的应用,促进质子泵抑制剂的合理使用。方法 借助处方前置审核系统的自维护规则库,针对医院门诊质子泵抑制剂不合理使用情况,对质子泵抑制剂的适应证、用法用量、重复用药、相互作用等进行规则设置,事前干预质子泵抑制剂不合理处方。比较用药规则设置前后门诊处方中质子泵抑制剂的处方合理率及各审核项下不合理处方率。结果 规则设置后,质子泵抑制剂处方合理率由76.31%提高到94.79%。适应证不适宜处方比例由22.01%降至5.14%,重复用药由0.66%降至0.07%,用法用量不适宜及相互作用的处方比例降为0。结论 质子泵抑制剂用药规则设置促进了医院门诊处方前置审核工作的顺利开展及质子泵抑制剂在门诊的合理使用。Objective To explore the application of proton pump inhibitor medication rules in the pre audit of hospital outpatient prescriptions,so as to promote the rational use of proton pump inhibitors. Methods With the help of the self maintenance rule base of the prescription pre audit system,in view of the unreasonable use of proton pump inhibitors in hospital outpatient clinics,the indications,usage and dosage,repeated medication,interaction and other rules of proton pump inhibitors are set to intervene the unreasonable prescription of proton pump inhibitors in advance. Compare the changes of reasonable prescription rate of proton pump inhibitor in outpatient prescription and unreasonable prescription rate under each audit item before and after the setting of medication rules. Results After the rules were set,the reasonable rate of proton pump inhibitor prescription increased from 76. 31% to 94. 79%. The proportion of prescriptions with inappropriate indications decreased from 22. 01% to 5. 14%,the proportion of repeated drugs decreased from 0. 66% to 0. 07%,and the proportion of prescriptions with inappropriate usage and dosage and interaction decreased to 0. Conclusion The setting of medication rules for proton pump inhibitors promotes the smooth development of pre audit of outpatient prescriptions and the rational use of proton pump inhibitors in outpatient clinics.
