右美托咪定混合罗哌卡因在大隐静脉曲张高位结扎剥脱术中的应用效果

摘　　要：目的:探讨右美托咪定混合罗哌卡因在大隐静脉曲张(VGSV)高位结扎剥脱术中的应用效果。方法:选取2021年11月-2022年3月福建省漳州市医院收治的行单侧VGSV高位结扎剥脱术患者60例,随机分为两组,每组30例。所有受试者均于喉罩全身麻醉诱导前行超声引导下股神经阻滞(FNB)。罗哌卡因组(R组)单一使用罗哌卡因,右美托咪定加罗哌卡因组(D+R组)使用右美托咪定加罗哌卡因。比较两组患者感觉和运动阻滞起效时间和持续时间,术后4、8、12、24 h疼痛程度,术后24 h不良反应发生情况及患者满意度。结果:D+R组感觉阻滞起效时间、运动阻滞起效时间均短于R组,感觉阻滞持续时间、运动阻滞持续时间均长于R组(P<0.05);D+R组各个时间点VAS评分均低于R组(P<0.05);D+R组不良反应发生率为20.00%,R组不良反应发生率为13.33%,两组差异无统计学意义(P>0.05);D+R组患者满意度为96.67%,高于R组的73.33%(P<0.05)。结论:对行VGSV高位结扎剥脱术患者以右美托咪定混合罗哌卡因在超声引导下FNB复合喉罩全身麻醉的方式进行干预,可有效缩短阻滞起效时间,不增加运动阻滞时间,可有效缓解疼痛,减轻不良反应,提高患者满意度。Objective:To investigate the application effect of Dexmedetomidine mixed with Ropivacaine in high ligation and stripping of varicose great saphenous vein (VGSV).Method:A total of 60 patients with high ligation and stripping of unilateral VGSV who were admitted to Zhangzhou Municipal Hospital of Fujian Province from November 2021 to March 2022 were selected and randomly divided into two groups,30 cases in each group.All patients underwent ultrasound-guided femoral nerve block (FNB) before induction of general anesthesia with laryngeal mask.Ropivacaine group (group R) was given Ropivacaine alone,and Dexmedetomidine plus Ropivacaine group (group D+R) was given Dexmedetomidine plus Ropivacaine.The onset time and duration of sensory and motor block,the degree of pain at 4,8,12 and 24 h after operation,the occurrence of adverse reactions at 24 h after operation and patient satisfaction were compared between the two groups.Result:The onset time of sensory block and motor block in group D+R were shorter than those in group R,and the duration of sensory block and motor block were longer than those in group R (P<0.05).The VAS scores of group D+R at all time points were lower than those of group R (P<0.05).The incidence of adverse reactions in group D+R was 20.00%,and the incidence of adverse reactions in group R was 13.33%,there was no statistically significant difference between the two groups (P>0.05).The satisfaction of patients in group D+R was 96.67%,which was higher than 73.33% in group R (P<0.05).Conclusion:The intervention of Dexmedetomidine mixed with Ropivacaine under ultrasound-guided FNB combined laryngeal mask general anesthesia in patients with high ligation and stripping of VGSV can effectively shorten the onset time of block without increasing the time of motor block,effectively alleviate pain,reduce adverse reactions,and improve satisfaction of patient.
