中药足浴联合常规清创换药对糖尿病足的疗效

摘　　要：目的:分析中药足浴联合常规清创换药治疗糖尿病足的效果。方法:选取2019年4月-2021年3月萍乡市人民医院84例糖尿病足患者,采用随机数字表法分为研究组(n=42)、对照组(n=42),对照组实施常规清创换药,研究组在对照组基础上联合中药足浴,比较两组治疗总有效率、治疗前后炎症反应情况、治疗后溃疡愈合面积情况、创面愈合时间及换药次数、治疗前后踝肱指数及下肢神经感觉阈值,统计两组不良反应发生情况。结果:研究组总有效率92.86%,较对照组的73.81%更高(P<0.05);治疗后两组血清C反应蛋白(CRP)、降钙素原(PCT)及白细胞计数水平均低于治疗前,且研究组均低于对照组(P<0.05);治疗2周后,研究组溃疡愈合面积大于对照组(P<0.05);研究组创面愈合时间较对照组短,换药次数较对照组少(P<0.05);治疗后两组踝肱指数均大于治疗前,且研究组大于对照组,下肢神经感觉阈值均低于治疗前,且研究组低于对照组(P<0.05);治疗期间两组均未见不良反应。结论:中药足浴具有和血通痹、益气温经的效果,联合常规清创换药可提高糖尿病足患者的治疗效果,促进溃疡愈合,抑制炎症反应,改善下肢缺血症状。Objective:To analyze the effect of foot bath with traditional Chinese medicine combined with routine debridement and dressing in the treatment of diabetic foot.Method:A total of 84 patients with diabetic foot in Pingxiang People’s Hospital from April 2019 to March 2021 were selected and divided into study group(n=42)and control group(n=42)by random number table method.The control group was treated with routine debridement and dressing change,and the study group was treated with foot bath by traditional Chinese medicine on the basis of the control group.The total effective rate,inflammatory reaction before and after treatment,ulcer healing area after treatment,wound healing time and dressing change times,ankle-brachial index and nerve sensory threshold of lower limb before and after treatment were compared between the two groups,the incidence of adverse reactions between the two groups were counted.Result:The total effective rate of study group(92.86%)was higher than 73.81%of control group(P<0.05).After treatment,the levels of serum CRP,PCT and white blood cell count in the two groups were lower than those before treatment,and those in study group were lower than those in control group(P<0.05).After 2 weeks of treatment,the ulcer healing area in the study group was larger than that in the control group(P<0.05).The wound healing time of the study group was shorter than that of the control group,and the frequency of dressing change was less than that of the control group(P<0.05).After treatment,the ankle-brachial index of the two groups were greater than those before treatment,and that of the study group was greater than that of the control group.The lower limb nerve sensory threshold of the two groups were lower than those before treatment,and that of the study group was lower than that of the control group(P<0.05).There were no adverse reactions in both groups during treatment.Conclusion:Foot bath with traditional Chinese medicine has the effect of relieving obstruction and improving temperature meridian,combine
