从痰瘀角度探讨中风的发病机制及治疗

摘　　要：中风病是一类临床的常见和多发疾病,在西医可归类为“急性脑血管疾病”,包括“缺血性中风”和“出血性中风”。自古代起,就有很多医家对于中风的诊治以“痰浊”“瘀血”因素展开论述。近五年以来,随着对中风疾病研究的不断深入,基于“痰、瘀”角度论治中风疾病的论述愈发普遍。导师在临床工作中同样发现“痰、瘀”两者已变成中风疾病的基本致病因素,“痰”与“瘀”两者既为病理产物、又为致病因素,相互促进。故治疗上也应注意痰瘀同治。笔者通过收集相关文献,简要论述古代与近现代的医家对于中风疾病中痰瘀理论的理解;通过整理相关临床试验研究,观察痰瘀并治方法对于临床疗效的影响;通过总结分析中风病各个时期的理法方药,来论述以“痰、瘀”角度论治中风疾病的近现代研究进展,以期为中风疾病的诊疗与研究提供一些思路。Stroke is a kind of clinical common disease and multiple disease,which can be classified as"acute cerebrovascular disease"in western medicine,including"ischemic stroke"and"hemorrhagic stroke".Since ancient times,many doctors have discussed the diagnosis and treatment of stroke from the perspective of the factors of"phlegm turbidity"and"blood stasis".In recent five years,with the deepening of the research on stroke,the discussion on the treatment of stroke based on"phlegm and blood stasis"has become more and more common.In clinical work,the tutor also finds that"phlegm and blood stasis"have become the basic pathogenic factors of stroke,and"phlegm"and"blood stasis"are both pathological products and pathogenic factors,which promotes each other.Therefore,we should also pay attention to the treatment of phlegm and blood stasis.The author,through collecting relevant literature,briefly discusses the understanding of Phlegm Stasis Theory in stroke diseases by ancient and modern doctors;Through the relevant clinical experimental research,the clinical effect of the treatment method of phlegm and blood stasis is observed;Through summarizing and analyzing the theory and methods of stroke in various periods,this paper discusses the recent and modern research progress of treating stroke diseases from the perspective of phlegm and blood stasis,in order to provide some ideas for the diagnosis and treatment of stroke diseases.
