围绝经期崩漏的诊疗进展

摘　　要：围绝经期崩漏是围绝经期的标志性症状,占围绝经期患者总量的70%,特指女性在围绝经期时发生在月经方面出现经血不止的疾病,可伴有潮热心烦、盗汗失眠等伴随症状。因其可导致严重贫血,故属于妇科急症。现代医学中异常子宫出血表现为月经的周期、经期、经量、频率、规律性中任何一项不符合正常的源自子宫腔的异常出血,因其表现与中医学中的“崩漏”相似,因此可参考“崩漏”进行治疗。西医常用激素药物治疗,包括孕激素、避孕药等。药物治疗效果不佳则选用手术治疗,排除子宫内膜结构异常。由于年龄的增长,避孕药需考虑有无潜在风险,应用激素进行治疗虽能达到一定止血目的,但是不良反应多,病情易反复。中医治疗围绝经期“崩漏”具有独特优势。文章简述了近年来围绝经期崩漏的中医辨证治疗进展,希望可以为临床医生提供诊疗思路。Perimenopausal uterine bleeding is a landmark symptom of perimenopausal period,accounting for 70%of the total number of patients in perimenopausal period.It refers to the disease of menstrual bleeding not stopping during menstruation,which can be accompanied by hot flashes,upset,night sweat,insomnia and other accompanying symptoms.Because it can lead to severe anemia,so it belongs to the gynecological emergency.Abnormal uterine bleeding in modern medicine is any abnormal menstrual cycle,menstruation,volume,frequency,regularity that does not meet the normal bleeding from the uterine cavity,because it is similar to the motherland medicine leakage,so we can refer to traditional Chinese medicine leakage for its treatment.Western medicine commonly applies hormone medicine treatment,including progesterone,oral contraceptives,etc..If the drug treatment effect is poor,surgical treatment is selected to exclude the abnormal endometrial structure.Due to the growth of age,oral contraceptives need to consider whether there are potential risks.Although the treatment with hormones can achieve certain hemostasis purposes,there are many adverse reactions,and the disease is easy to relapse.TCM treatment has unique advantages.This paper summarizes the progress of TCM syndrome differentiation treatment in recent years,hoping to provide diagnosis and treatment ideas for clinicians.
